Novelist Marian Keyes reveals fight against constant "suicidal impulses"

The novelist Marian Keyes has revealed how she battled constant suicidal urges at the height of her mental illness.


The Irish writer opened up to Kirsty Young about her struggle with alcoholism and depression on BBC Radio 4’s Desert Island Discs on Sunday.


At her worst – an 18-month period beginning in 2009 – Keyes said she struggled not to harm herself.


She had stopped being able to eat, sleep and interact with others, and was eventually hospitalised.


“Then the suicidal impulses started and it was very hard to physically to stop myself from going through with it,” she said. “For months and months, every day was an enormous effort not to do the acts of wounding myself.”


Her recovery was “really speedy”, but the experience altered her perception of what it meant to be depressed.


“Nothing worked but the passage of time … It’s an illness and it ran its course.


“I had always described myself as melancholy or depressive but I hadn’t a clue. Anything I had before was a blue day by comparison. This was altered perceptions, a mental illness.”


She wrote her novel The Mercy of Mystery Close in that period. She told the Guardian in 2013 that a scene in which the heroine planned to kill herself in a hotel room was inspired by her own experience.


“I had two goes going out assembling the whole kit and buying paper and Sellotape to write the note,” she said. “The conversation Helen has with the man in the shop, I actually had that, with him asking: ‘What is it you’re proposing to cut?’


“It was so bizarre to be standing in a DIY shop, buying a knife to open my veins with. I was absolutely going through who would find me, leaving money for her to apologise … I wasn’t in my right mind.”


Keyes, 53, also told Young about her experience of alcoholism while living in London in her 20s.


“Alcohol was the love of my life. It was my best friend and, in the end, my only friend … I had stopped eating, stopped hoping, I was constantly suicidal. There was only one way it would go. I couldn’t stop drinking and was preparing to go under.”


Writing was her “rope across the abyss”: she wrote her first short story, then a few months later began a rehabilitation program for alcoholism. She has now been sober for more than 20 years.


“It was that primal urge in all of us to stay alive, saying ‘I can give you this, will you live for this?’ It didn’t get me sober but gave me something to hope for.”


Keyes’ debut novel, Watermelon, was published in 1995.


Her 12 novels to date have sold 35m copies worldwide and been published in 33 languages.


Her 13th book, The Break, will be released in the UK and Ireland on 7 September.

New weapon in the global fight against fake malaria drugs: a cheap scanner

A new device that uses similar infrared light to TV remotes can accurately detect fake antimalarial drugs, according to a scientific paper published Monday.


The researchers revealed how they were able to use an optical scanner purchased online for $ 250 to distinguish perfectly between life-saving malaria drugs and deadly counterfeits.


Dozens of public health scientists declared in 2015 that a global crisis of fake drugs was undermining the fight against malaria, tuberculosis and HIV/Aids, particularly in the developing world.


The World Health Organization estimates that falsified medicines represent more than 50% of the pharmaceutical market in several African countries. Ineffective antimalarial drugs alone killed over 120,000 preschool children in Africa in 2013, according to research from the Center for Disease Dynamics.


“We’ve talked to several NGOs and government agencies who would like to do drug quality screening but can’t because they don’t have effective tools,” said Ben Wilson, a research scientist at Global Good, a collaboration between Bill Gates and the technology company Intellectual Ventures.


Wilson’s team, together with researchers at the London School of Hygiene and Tropical Medicine (LSHTM), set out to design an easy-to-use, portable scanner that almost any charity or rural pharmacy could afford.


Many fake drugs are almost indistinguishable from the genuine products, even down to convincing anti-counterfeiting holograms on their packaging. Testing the drugs currently requires laboratory tests with machines costing many thousands of dollars, operated by skilled technicians.


One testing process – spectroscopy – involves shining a light on a material, then analysing the light that comes back. Precise, powerful lasers allow researchers to identify every chemical in a pill, so they can determine which ones contain sufficient artemisinin – the active ingredient in most modern antimalarials.


But Wilson opted for a more rudimentary approach. He bought a spectrometer called Scio from the Israeli startup Consumer Physics, which had crowdfunded the production of the handheld device on Kickstarter. Instead of a laser, Scio uses a cheaper LED light – essentially a souped-up version of the infrared LED in a TV remote.


While infrared spectroscopy cannot pick out the individual drugs that make up a pill, it can capture a medicine’s overall spectral fingerprint. Wilson’s team scanned genuine drugs with sensitive lab equipment, then used machine learning to extract a unique algorithm for each.

Scio connects to a smartphone app that compares those fingerprints to a sample in front of it.


A paper published in the American Journal of Tropical Medicine and Hygiene described lab tests of over 900 samples of antimalarial drugs purchased in Equatorial Guinea and Ghana, in which Global Good’s system detected every single fake.


Global Good and LSHTM want to get handheld scanners like Scio out to workers in the field, where they would connect with a smartphone app the organisation has also developed. Anyone from customs officials to aid workers would be able to scan a drug in seconds, getting an instant readout from the phone whether a medicine is genuine or not.


“Everything is hosted on the phone itself,” said Wilson. “This has to work in remote locations in India or Africa where there is no data service.”


Muhammad Zaman, professor of biomedical engineering and international health at Boston University, agreed that cheap, reliable scanning is essential. However, he argued that substandard drugs, whether the result of poor quality manufacturing or expired supplies, are as much a concern as counterfeit drugs.


“People envisage a mafia working in shadows but the problem is more complicated than that,” he said. “Sometimes good manufacturers make bad drugs because there is no regulation or quality oversight.”


Zaman is developing his own system, called PharmaChk, that squeezes an entire chemical lab into a suitcase. Unlike Global Good’s spectrometer, it destroys the pills it tests, requires a supply of chemicals to operate, and is likely to cost $ 5,000 or more. “But we can tell exactly how much artemisinin is in a sample,” he said.


Global Good’s system has trouble spotting some substandard antimalarials because they still carry the fingerprint of a reputable manufacturer. However, the organisation is already working with a more sophisticated scanner that should be better at identifying poor quality drugs and can even scan pills through a blister pack. If a trial of the new scanner in Laos goes well, Global Good and LSHTM hope to interest a large funding agency in rolling out the technology.


The US Food and Drug Administration is also developing its own handheld scanner to detect counterfeit medicines, and one Chinese company is even building infrared spectroscopy into an Android smartphone.


Ultimately, everyone could one day have the technology to check medicines in the palm of their hands. “The big effort is collecting the data,” Wilson said. “The way our system works, there’s no limit to how many drugs you can scan.”

Dozens of children who developed narcolepsy as a result of a swine flu vaccine could be compensated after the high court rejected a government appeal to withhold payments.


Six million people in Britain, and more across Europe, were given the Pandemrix vaccine made by GlaxoSmithKline during the 2009-10 swine flu pandemic, but the jab was withdrawn after doctors noticed a sharp rise in narcolepsy among those who received it.


The sleep disorder is permanent and can cause people to fall asleep dozens of times a day. Some narcoleptics have night terrors and a muscular condition called cataplexy that can lead them to collapse on the spot.


In 2015, a 12-year-old boy, known as John for the proceedings, was awarded £120,000 by a court that ruled he had been left severely disabled by narcolepsy caused by the vaccine. He was seven when he had the jab and developed symptoms within months.


Because of his tiredness, John became disruptive at school and found it almost impossible to make friends. He takes several naps a day, cannot shower or take a bus on his own, and may never be allowed to drive a car.


Despite paying out, the Department for Work and Pensions argued John’s disability was not serious enough to warrant compensation and said the court was wrong to take into account how the illness would affect him in the future. But the high court on Thursday rejected the government’s appeal that only the boy’s disability at the time should have been considered.


The ruling paves the way for more than 60 other people to claim compensation.


“This important decision brings clarity to anyone who brings claims under the Vaccine Damage Payment Act in future,” said Peter Todd, the family’s solicitor at Hodge Jones & Allen. “It will in particular bring welcome relief to those who developed narcolepsy as a result of taking the swine flu vaccination and who have been awaiting payment from the DWP scheme but also has implications for anyone affected by other vaccines covered by the scheme.”


The judgment means the DWP has to take into account the impact disability has on a person’s entire life, and not just the impact it has on the individual at the time their claim is made.


“Sadly, those who developed narcolepsy as a result of the swine flu vaccination have had their lives changed forever. The condition will affect many aspects of their lives including working, driving, personal and family relationships – the very things most of us take for granted,” Todd said.


In 2014, a 23-year-old nursery assistant who developed narcolepsy after receiving the swine flu vaccine took her own life, telling her family that living with the sleep disorder had become unbearable. In a note written on the day she died, Katie Clack, urged her family to pursue her legal case, saying she had been left with “no quality of life”. Her sister, Emma Sutton, told the Guardian at the time: “We feel she was let down by the defective vaccine, which caused her narcolepsy, and by the insufficient intervention and support, which ultimately led to this tragedy.”


The Pandemrix compensation case was the first that the court of appeal considered under the Ul statutory compensation scheme, which was set up in 1979 for rare occasions when vaccines cause severe damage. The decision to consider the impact of the disability over the person’s entire life is now binding on all future cases brought under the act.


Todd said there are about 100 people in the UK with narcolepsy caused by Pandemrix. A further 100 applications a year are made for compensation under the scheme due to harm caused by other vaccines. “Today’s judgment brings a welcome relief to the many people affected by the DWP’s continued refusal of applications for compensation,” Todd said. He is acting for 88 claimants, mostly children, who developed narcolepsy as a result of the swine flu vaccine, and in a civil case against GSK, which manufactured the vaccine.


A DWP spokesperson said: “We are aware of the judgment of the court and are carefully studying the court’s reasons.”

Women’s Health and Weight Loss: Lose Weight to Fight Osteoporosis

It is a well-known fact that exercise and weight loss make you both look and feel better. Another bit of common knowledge is that to lose weight, control your diet, and exercise regularly gives you a healthier heart.


In addition to the previously mentioned benefits of proper diet and exercise are several other very specific health benefits of having a regular exercise and weight loss program.


One additional health benefits of exercise and weight loss activities is guarding against osteoporosis, the disease that makes the bones weak and brittle with age (1).


Weight-bearing activities like walking, elliptical machine, and jogging, have been shown to strengthen the skeletal system and lower the risk of the development of this very debilitating disease.


The primary reason for this is that exercises that force the body to bear weight make the bones denser over time as opposed to a sedentary lifestyle, in which the bones get weaker and more brittle due to a lack of force placed against them.


In women, the low estrogen levels that often accompany menopause and/or very sporadic menstruation make the problem even worse as the bones lose a lot of their mineral content and weaken accordingly.


In addition, such activities that many take part in to either lose weight or maintain an already healthy body composition have also been shown to aid in preventing further development of the disease for those who have already been diagnosed and have received clearance from their doctor.


  1. Calcium and bones

So, in addition to exercise and hormonal factors, a woman’s diet is extremely important in guarding off this very debilitating condition.


One of the reason why we do not recommend most traditional diet plans and programs is because the over-restrictive eating guidelines often eliminate or seriously limit the consumption of some very necessary vitamins and minerals that keep the body healthy and strong.


The most important time to get an appropriate amount of calcium is between the ages of 14 and 24, when peak bone mass is obtained.  Then after that bone continues to develop until about age 35 to 40, at which time the bone mass that a woman has will strongly determine how much at risk she may be for bone fractures in her later years.


The National Institutes of Health (NIH) recommends that women intake 1,000 mg (1g) of calcium daily, and then increase their daily dosage to 1,500 mg (1.5g) post menopause (2).


Sadly, only an estimated 25% of women in any age group consume the recommended daily amount of calcium to guard against osteoporosis. And when they go on diets and weight loss programs, they tend to consume even less. The best sources of easily assimilated calcium are raw milk and bone broth.


  1. Iron and Bones

Finally, one of the biggest weaknesses of the traditional commercial diet is the lack of iron that exists in its stringent and often excessively limited eating guidelines (3). In order to lose weight, many unsuspecting women get bamboozled into taking part in weight loss programs that cut out many of the nutrients that they need. Iron is one of the primary casualties of such diets and weight loss plans.


The plot thickens for women in particular due to the fact that their bodies regularly lose iron during menstruation.  Therefore, eating a low-calorie, low-iron diet in an effort to lose weight only makes the problem more severe.


It is generally suggested that women supplement their diets with an iron supplement such as either Ferrofood or Iron-Fe Max from Standard Process. This is particularly important during periods of heavy exercise and training that are typical aspects of any weight loss program or general effort to lose weight even when a special program is not in use.


One thing to be on guard for, however, is excessive iron supplementation by iron-deficient (non-anemic) women.   There is a disorder known as hemosiderosis which results from large deposits of iron made in the liver that causes a glitch in the proper metabolism of the iron itself. Avoiding prolonged large doses will eliminate the risk of this particular disorder. Supervision by a trained and experienced holistic doctor is recommended.


Below are a few recommendations from the American Council on Exercise (ACE) that will help you to make sure you get the adequate amount of iron intake in your daily diet, whether on a program to lose weight or not:


Eat foods with a high vitamin C content with all meals. Vitamin C aids in the absorption of iron. (examples: salsa; chili peppers; oranges)


Include dark meat chick and/or turkey in the training diet. Both of these are rich in iron.


Vegetable proteins such as split pea soup or chili beans with lean meat help the iron in those meats to properly absorb

2016 was the worst year in NHS history – we must fight for its survival

The last 12 months have been the worst in the history of the NHS. Our health system is under pressure like never before. The moment of crisis many warned of has arrived, and it is not clear that the NHS can be retrieved from this state of affairs.


We used to say that flailing A&Es represented an early warning sign that the health service was under pressure. And so that has proven to be. England’s major A&Es are under record strain with black alerts being regularly sounded, and in some instances wards turning patients away. Last year the A&E crisis spread to other sectors.


Ambulance response times have reached critically low levels, with one third of ambulances failing to meet their targets for life threatening callouts. The acute care sector is bulging with unnecessary admissions particularly from over-75s who are presenting themselves at A&Es when they should be cared for by the social care sector that has suffered £5bn of cuts.


In the community, general practice is on life support; as more is demanded of it, the proportion of the NHS budget that goes to primary care has effectively shrunk. Primary care provides 90% of the consultations in the NHS yet only gets 8% of the budget. GPs are leaving, and new entrants are declining to enter general practice.


We once bickered with the Treasury for clawing back millions in Department of Health underspend under Labour. Under the Tories, that is a distant memory as NHS trusts recorded a deficit in excess of £2bn last year.


A government elected to fix near-bankrupted banks has replaced that by bankrupting our hospitals. Idiotic spending decisions in the NHS have been rife.


The most worrying aspect of the government delivering the lowest additional funding increase to the NHS in its history has been the knock-on effect on patients, in terms of treatment and facilities available. More than 13,000 beds have been closed, cutting the capacity of the NHS by 5 million a year.


So bad was 2016, that nine former health secretaries condemned the government for failing to live up to its promises on mental health.


Commissioners and providers alike have had to resort to rationing care to try to balance their books. Unfortunately, this is not always to the benefit of customers or patients.

Given that the private sector has a stated goal to make 8%-14% profits from the NHS, can taxpayers really afford this choice?


While the NHS has increased the cash it takes from the private sector by 30% to £558m last year (pdf), waiting lists have soared to an eight-year high. Now, 4 million patients are on waiting lists. This in effect means NHS patients are being delayed in their treatment to make way for wealthy private patients who can afford to skip the queue.


The pressure on our staff has also reached unprecedented levels. Nurses have seen real-term pay cuts since 2010 of over £2,000. Moreover, aspiring nurses will also be denied a bursary to train and this at a time when unfilled nursing vacancies have climbed 600% since 2010. It will not surprise you to learn that applicants for trainee nursing courses have fallen 20% this year.


Likewise, the pressure on junior doctors remains intense. They too have seen real-term pay cuts, as well as an enforced contract.


It remains impossible to meet the demands of the ill-described seven-day NHS until serious funding issues have been resolved, otherwise we are asking our doctors to shoulder the blame for unsafe care.

Let’s pledge in 2017 to fight for its survival. The NHS belongs to us, not the politicians and not the privateers. We cannot trust the government to be its safe custodian. It’s up to each and every one of us to fight for the NHS, otherwise it won’t be there to look after us, never mind the next generation.
