The NHS plan for the next two years represents a perceptible contraction of the health service’s offer to the public.


The proposals in Next steps on the NHS Five Year Forward View, published on Friday, are shaped by shortages of money and staff.


Simon Stevens, the NHS England chief executive, has burned through much of his political capital in disputing government claims about whether the NHS has been given all the money it asked for, so this was not an opportunity to push for further cash.


So in the face of the unrelenting pressure of the government’s austerity programme and barely controlled hospital debt, he is gambling that politicians and the public will stomach longer waits for routine surgery if the health service can deliver better performance on cancer treatment, A&E waits, mental health services and GP appointments.


In the wake of slipping cancer treatment times and the recent outcry over the death of a child waiting for urgent surgery, this is probably the right choice. But the price could be high.


Allowing elective surgery times to slide over many years was what led to hundreds of thousands of patients waiting months for operations by the time New Labour came to power in 1997, elected partly on its pledge to slash waiting lists. It would be a serious blow if the NHS returned to the days of people dying while waiting months for heart surgery, and many more forced to endure avoidable pain and disability.


Access to some of the latest approved drugs is also to be delayed – in breach of a commitment in the last Conservative manifesto to speed up access.


Stevens is anxious to reassure the public that performance will not slide back to that of the 1990s, but it is difficult to see how growing waiting times will be arrested and reversed in the coming years.


The plan makes some brave assumptions about the ability of the NHS to expand its workforce, including 4,000 more nurses through improving staff retention – turning round a recent trend – and up to 2,000 more nurses returning to practice.


There is yet another pledge to increase the number of GPs substantially, despite little discernible progress. However, concerns are growing that staff shortages will be exacerbated by EU staff heading back to the continent in the wake of the Brexit vote.


More promisingly, the plan may well mark the beginning of the end of the internal market. It names nine areas being considered as pioneering “accountable care systems”, with NHS organisations and local authorities working together as an integrated health system.


These areas will have considerably more control over how they deliver their healthcare, and will be effectively freed from the endless focus on contracts rather than patients imposed by the purchaser/provider split.


However, doing so will require ever greater legal contortions to simultaneously stay within the law while circumventing it. This is a necessary bodge, as it will be years before anyone attempts another NHS reform bill, but these workarounds cannot be sustained indefinitely.


Everyone will now be working in Sustainability and Transformation Partnerships, and NHS England makes clear that anyone who fails in their duty to collaborate can expect to be handled roughly.


In the absence of more money, the alternative to Stevens’ plan would be a steady atrophying of performance across the entire health service. Instead, NHS England is pushing forward on the key priorities of emergency care, cancer treatment, mental health and primary care, and allowing routine surgery to pay the price.


But many of its promises look optimistic, and there is a danger that the slide in surgical performance will eventually reverse years of progress.


For the time being this can be portrayed as a tactical move, but with austerity set to last well into the next decade, there is a risk that the NHS will cease to be a comprehensive service.

Is Lymph Node Removal the Safest Choice for Breast Cancer?

If you have been diagnosed with breast cancer, your oncologist may suggest lymph node removal as part of the course of treatment. Whether to get this procedure done is a personal choice that each woman must make. It is important to know the risks—and your options– before you commit to this potentially-dangerous procedure, however.


Why is lymph node removal so common?


When a woman is diagnosed with breast cancer, often times “sentinel lymph nodes” around the affected area may be removed via biopsy before conventional treatment begins (other times they are removed during surgery). Conventional oncologists often suggest this based on the premise that cancer cells tend to spread first to “sentinel lymph nodes,” i.e. the lymph nodes closest to the tumor, before they spread to “auxiliary nodes” and the rest of the body.


Lymph node removal in breast cancer patients has been around since the early 1900’s; it is part of an old paradigm that focuses primarily on “getting all of the tumor out.”  As new ways of looking at cancer are come to the forefront, however, even researchers and doctors within the halls of conventional medicine are beginning to question whether the procedure is necessary for all women.


The field of breast cancer oncology was turned upside down in 2011 when a study out of John Wayne Cancer Center in Southern California announced that for roughly 20% of all U.S. breast cancer, lymph node removal did not make any difference whatsoever in their recovery. For approximately 40,000 women per year who have stage T1 or T2 cancer tumors that have not traveled elsewhere in the body, it did not improve their survival nor their metastasis rates in any way.


“I have a feeling we’ve been doing a lot of harm,” said Dr. Grant W. Carson, one of the authors of the report, in reflecting on the legacy of lymph node removal for breast cancer patients that has persisted until this point.


The Perils of Lymphedema


Lymph nodes are vital for the body for several reasons. They are one of the areas where white blood cells and Natural Killer Cells are produced. The lymphatic system is also the body’s second most important line of defense against harmful pathogens (besides the skin). It filters unwanted substances that may accumulate throughout the whole body to detoxification pathways so that they can be eliminated.


For those who have a Sentinel Lymph Node Biopsy (SLNB), an estimated 5-17% will develop lymphedema, according to the National Cancer Institute. For Auxiliary Lymph Node Dissection (ALND) surgery, the percentage jumps to between 20 to 53 percent. And a whopping 70% of those who elect to have all of their breast-area lymph nodes removed will develop the condition.


Lymphedema is a painful condition that happens when lymph fluid becomes clogged and collects under the skin’s surface—and edema and fibrosis can follow if it is not addressed. Fibrosis in particular stops the natural flow of oxygen and healthy substances through the lymph system and can lead to bacterial overgrowth and infection.


3 Easy Ways to Keep Your Lymph System Flowing…Naturally


If you have already had some or all of your breast area lymph nodes removed as part of a conventional breast cancer protocol (or are prone to lymphedema for other reasons), be sure to add these important practices to your overall health routine:


  • Move your body! This is perhaps the most important thing you can do to keep your lymph and detoxification pathways healthy. Remember that lymphatic fluid needs to flow and regular exercise can help that process. A 2011 review of 11 other studies published in the Journal of Cancer Survivorship found that “strong evidence is now available on the safety of resistance exercise” for breast cancer patients.” Resistance exercises for lymphedema would include gentle exercises that lightly use your muscles against an opposing force, such as a resistance band. Many experts also claim that “rebounding” (using a small trampoline) is also one of the most effective exercise modalities for both the lymph nodes and the immune system.

  • Consider ancient Ayurvedic lymphatic modalities such as dry skin brushing and traditional and as well as modern versions of lymphatic massage;

  • Finally, assist your detoxification process in general by staying hydrated with lots of fresh, filtered water and also by staying away from toxins that may come in contact with the skin, such as commercial deodorants which contain aluminum.

‘Stark choice’ as schools struggle to foot bill for poorer students

Kevin Prunty is executive head teacher at Cranford community college, a high-achieving school in Hounslow, west London. His pupils are ambitious and successful, but many come from disadvantaged backgrounds.


Like other schools serving deprived areas, Cranford find itself increasingly playing a sophisticated welfare role in its community. It is also footing the bill for uniforms, PE kits, shoes, lunches and educational trips from a diminishing budget, to subsidise parents who cannot afford to meet the costs.


But with school budgets under pressure and further cuts expected there are fears they will not be able to continue to fill the gap.


“Schools know already that there are sizeable further cuts to funding on the way – and whilst we are currently able to fund these additional needs – it will soon become more difficult and perhaps impossible to justify doing so,” says Prunty.

He says it’s not possible to measure the additional cost in the sense of how many uniforms, how many trips, how many meals. Instead he describes the kind of scenarios his staff deal with on a daily basis to illustrate the level of need.


A year 7 boy needs additional support but his symptoms don’t meet the threshold to qualify for child and adolescent mental health services (Camhs).


An educational welfare officer helps a family where the father is suffering from a mental illness and keeps telling his children they are terminally ill. Another colleague intervenes to help a child with impaired hearing.


Someone else takes a pupil out to buy a suit for a job interview for a traineeship. It’s not just the money – they coach them for the interview and make sure the suit fits properly. “There’s nobody to say whether the arms are too long, so some of our staff take the responsibility,” says Prunty.


And things are getting worse. Since pupils returned to school in September, staff have noticed an increase in the number of children who are not eligible for free school meals who come in without any money for lunch. Inevitably the school provides – it all costs and meanwhile budgets are shrinking.


The Institute for Fiscal Studies has estimated that increasing costs and growing pupil numbers will result in an 8% cut in funding per student by 2020 and an inquiry has been begun by the public accounts committee to examine the financial sustainability of schools.


The sector would like to hear there is more money for schools in the autumn statement later this week – the reality is that schools like Prunty’s are likely to lose out in the long-awaited redrafting of the national funding formula (NFF) in which money from well-funded urban schools working with disadvantaged families is expected to move to less well-funded schools in less needy areas.


He has already had to address a £1m shortfall in the budget caused by a cut in grant funding from the government and rises in pension costs, national insurance costs and below-inflationary salary rises. He has managed to balance the budget but twelve jobs had to be cut – four teachers and the rest support staff.


At another school in the area, a principal who asks not to be named says he increasingly feels like a debt collector, having to chase up parents who owe the school money. As families at his school struggle to make ends meet, many are building up large debts. Some owe hundreds of pounds for school lunches and he estimates that parental debt is costing the school £10,000 a year.


Last year the school handed out 45 uniform grants worth £100 each to parents in need. Then there are equipment grants for things like scientific calculators. He feels for the parents, many of whom are in low paid jobs and are surprised to find they are not entitled to free school meals for their children. “There’s a lot that’s adding to pressure on parents. It’s uncomfortable to be talking to them about the money they owe the school. But that money is money we are not spending on teachers,” he says. “Schools are not in the debt management business. It’s not sustainable to be running this level of debt.”


The issue has been raised by Seema Malhotra, Labour MP for Feltham and Heston, who has become increasingly concerned about the hidden burden on schools’ budgets. “Schools are being faced with a stark choice,” she said. “Let children from poorer backgrounds miss out on educational opportunities and experiences, or pick up the costs and take on financial debt. Budgets are already under pressure and with further cuts to school funding, helping families in need is set to get harder.”


At Springwest Academy in nearby Feltham, which also serves a community with high levels of disadvantage, there are similar concerns. Pastoral mentors deal with calls day in and day out from families with worries about housing and finances; school uniforms and shoes are being paid for more frequently out of the school’s hardship fund and almost four out of ten pupils (38.3%) are referred for counselling or other mental-health support.


Victoria Eadie is chief executive of the Tudor Park Education Trust which includes Springwest. “It does worry me what’s going to happen. I don’t know a school in Hounslow that is not having to go though some form of restructuring and quite severe cuts.” Until now her school, like others, has been able to step in to support the neediest. With further cuts, she says, “schools will not be able fill the gap”.

Sergentanis and researchers from the University of Athens, in Greece, conducted a meta-analysis comparing the effect on breast cancer risk of individuals who have used antibiotics versus those who have never used antibiotics. They found that those who used the medications had a slightly higher risk in developing breast cancer. The link is thought to be pointed at the weakening of the immune system, and the killing of gut-friendly bacteria, due to antibiotic use.


Although antibiotics can be helpful in treating more severe bacterial infections, perhaps doctors should try and offer patients more natural alternatives to treating the less severe ailments.

Aon Hewitt, the largest manager of a private insurance exchange, says that the approach reduced growth in health insurance costs for its clients’ active employees to 5 percent, compared to 7 percent for most large employers.


So far, Walgreen has been the largest company to disclose participation by workers in its private exchange. The pharmacy giant says more than 220,000 workers and their dependents are getting coverage through the Walgreen proprietary “Live Well Benefits Store,” a marketplace that is an outsourced solution through Aon Hewitt Corporate Health Exchange.


Aon Hewitt, Mercer and Towers Watson (TW) are the leading consultancies pushing the exchange approach for both active employees and retirees with IBM (IBM), AT &T (T) and Time Warner moving their workers to retiree private exchanges. And the consultants are on the verge of having more competition as health insurance companies get into the act like Cigna (CI), Aetna (AET) and others.


“We continue to work to develop our own proprietary exchange called, the Aetna Marketplace, which we believe will offer an attractive alternative to existing multi-carrier exchanges and the current defined benefit model for health benefits,” Aetna chairman and chief executive officer Mark Bertolini told analysts and investors Tuesday on the company’s second-quarter earnings call.
