NHS and medical watchdog tried to suppress scandal over vaginal mesh implants

NHS bosses and the watchdog that oversees medical devices tried to limit public exposure of the scandal over vaginal mesh implants that have harmed hundreds of women.

Minutes of a meeting held in October 2016 show that NHS England and the Medicines and Healthcare Products Regulatory Agency agreed to “avoid media attention” over the implants, despite the fact they were seeking to encourage patients to report any complications.

The document, obtained by the Press Association, records an agreement to “take the press element out” of the “yellow card” campaign to record adverse reactions experienced by vaginal mesh patients, suggesting that it could be folded into a wider effort, “of which mesh is one element, to avoid media attention on mesh”.

The apparent cooperation between NHS England and the MHRA to minimise media focus on the debilitating problems increasingly associated with the implants appears to breach the NHS’s duty – reiterated regularly by health secretary Jeremy Hunt – to be open and transparent over patient safety failings.

NHS England and the Department of Health both refused to comment on the minutes of the meeting.

One possible reason for the NHS to want to limit exposure of the issuecould be to reduce the number of potential lawsuits faced by the health service.

More than 800 women are suing the NHS and the manufacturers of vaginal mesh implants after suffering serious complications, it emerged this week. Some women reported that implants had cut into their vaginas, with one woman saying she was left in so much pain that she considered suicide. Others have been left unable to walk or have sex, according to the BBC.

Vaginal mesh implants are used to treat incontinence after childbirth or pelvic organ prolapse, where the womb or bladder bulge against the walls of the vagina. Between 2006 and 2016, more than 11,000 women in England were given the implants to treat prolapse or incontinence, NHS data shows.

Around 11%-12% of users have reported problems, while lawsuits in the US have already seen around $ 2bn (£1.5bn) paid to affected women.

Campaigners say that hundreds more women have come forward after learning of the group planning to sue.

Kath Sansom, who runs the campaigning website and Facebook group Sling the Mesh, says the number of women contacting her has risen from a few people a day to more than 200 in the past 24 hours.

“It’s always the same story,” she said. “There are so many women who were told it was just them, that they were a one-off. They can’t believe there are others out there. So many people are told it’s back pain, endometriosis, gall bladder pain, scar tissue. And so many of them accept it, you trust medical professionals.”

Data from the MHRA, which has been looking at the issue since 2011 following complaints from women, shows more than 1,000 adverse incidents have been reported in the past five years.

Despite the problems that have emerged the MHRA insists that the best current evidence supports the continued use of the implants to resolve health conditions that could themselves cause serious distress to patients.

A report into the issue from a working party led by NHS England admits there is a huge lack of data on complications from the devices. Published studies on mesh implants do “not tell the whole story” and there are gaps in NHS knowledge about their safety, it added.

NHS data loss scandal has prompted five inquiries, ministers say

The NHS’s loss of more than half a million pieces of confidential medical correspondence is so serious that it has triggered five separate investigations, ministers have admitted.

The disclosure has prompted claims that the scale of the loss of 515,000 test results and doctors’ letters was hidden in “a huge cover-up”. It raises fresh questions for the health secretary, Jeremy Hunt, who has publicly championed openness in the NHS.

The National Audit Office (NAO) and Information Commissioner’s Office (ICO) are currently looking into the incident, which the Guardian revealed last month, according to parliamentary questions that the health minister Nicola Blackwood answered on Friday.

Their investigations follow three other inquiries already undertaken by the Department of Health (DH), NHS England and NHS Shared Business Services (SBS), Blackwood said.

SBS is the private firm, co-owned by the DH, which mislaid the highly sensitive correspondence between 2011 and 2016 by putting items in a warehouse and forgetting to send them on to 7,700 GP surgeries across England despite their importance for patients’ health and treatment.

Labour and the Liberal Democrats have intensified their claims that the DH has been highly evasive about the scandal after Blackwood said the DH would not publish the results of any of the three investigations already completed.

“It is totally incredible that the secretary of state failed for so long to identify this catalogue of errors, and it beggars belief that Jeremy Hunt is refusing to publish the advice which he received,” said Jonathan Ashworth, the shadow health secretary. “This is a major scandal, and Jeremy Hunt needs to come clean with the public.”

Tim Farron, the Liberal Democrat leader, criticised the DH for “really disturbing” secrecy. “The fact that five separate investigations have been launched shows that the government and NHS England both knew that this was a monumental blunder,” he said. “But Jeremy Hunt’s attempt to downplay the severity of it, when all this was happening, smacks of a huge cover-up.”

The Guardian’s disclosure forced Hunt to answer an urgent question in the Commons on 27 February, when he downplayed the seriousness of the incident. But the DH’s own permanent secretary, Chris Wormald, confirmed to MPs on the public accounts committee on the same day that doctors were still examining whether more than 500 patients may have come to harm as a result.

Medical investigators are also looking into the possibility that the incident contributed to some patient deaths.

Hunt’s behaviour is under scrutiny after he admitted to MPs that he was first told about the data loss on 23 March 2016, but did not acknowledge it publicly until 21 July. Even then he only did so in a 138-word statement on the last day of parliamentary business, and mentioned neither the huge number of documents involved or the possibility that patients may have been harmed.

The DH’s refusal to publish the results of any of the three inquiries means that Hunt is avoiding scrutiny of what he knew and when, and what he did or did not do as a result of the documents being mislaid..

In the parliamentary questions Blackwood defended the decision not publish. “Given that this national incident is currently subject to an investigation by both the NAO and the ICO, it is not appropriate to publish related documents until these investigations have concluded,” she said.

NHS tainted blood scandal: scheme to compensate victims to be scaled back

A scheme to compensate the victims of an NHS blood contamination scandal is to be scaled back under government plans announced on Monday.

Ministers believe the reforms are necessary because more people are now considered likely to develop serious health issues – and be entitled to higher payouts – pushing the programme as much as £123m over budget.

The government has proposed measures that would cut predicted costs, including limiting the availability of the higher level of financial support under the scheme.

Campaigners said they had significant concerns about the proposed changes. While the Haemophilia Society welcomed the greater clarity, it said it was not confident that the government would achieve its aim of ensuring that no one who received an annual payment would be left worse off.

The scheme was set up to compensate the victims of the tainted blood scandal in the 1970s and 80s, during which thousands of haemophiliacs were mistakenly given blood contaminated with hepatitis C.

“In July 2016, estimates suggested that the reform would be affordable,” the government’s review said. But the Department of Health now believes that more people with pre-cirrhotic hepatitis C would benefit from the scheme’s higher tier of payouts than previously forecast. “Under new assumptions, the reforms would lead to an estimated overspend of between £76m and £123m,” the department’s impact assessment says.

In a bid to cut costs, the health minister Nicola Blackwood outlined plans to limit the highest level of financial support under the new scheme to those infected with hepatitis C who have developed advanced liver disease. A £50,000 lump sum would no longer be offered to those with less advanced infections.

Other changes include dropping plans to introduce a fixed increase in annual payments and reducing the budget available for discretionary payments.

In a written ministerial statement, Blackwood said: “The government does not anticipate that there will be any reduction in current spending as a result of the consultation proposals. No one who currently receives an annual payment will be worse off than they are now as a result of the proposed changes to the annual payments.”

In July, the government unveiled plans to add £125m of funding to its compensation scheme, more than doubling the DoH’s annual spend on the scheme to 2021. That included the new special appeals mechanism for those with chronic hepatitis C infection.

The six-week consultation runs until 17 April.

South African scandal after nearly 100 mental health patients die

At least 94 patients with mental health issues died after South African authorities moved themfrom hospitals to unlicensed health facilities that were likened to concentration camps, a government investigation has revealed.

Many of the deaths were due to pneumonia, dehydration and diarrhoea as the patients were hurriedly moved to 27 “poorly prepared” facilities in an apparent cost-cutting measure that showed evidence of neglect.

The health ombudsman report, which has sparked uproar, detailed how some patients were collected from the Life Esidimeni hospital in Gauteng province last year using open pickup trucks.

As the scandal broke, the provincial health minister, Qedani Mahlangu, resigned over the findings, which directly implicated her in the move.

According to the report, which was reportedly compiled after 80 hours of listening to family members and inspectors during the investigation – relatives were left in the dark over where the patients were, in unheated centres that some witnesses said were like concentration camps.

The centres also failed to provide seriously ill patients with enough food and water, leaving them severely malnourished, underweight and in some cases dying from dehydration.

Gauteng’s provincial health department had terminated its longstanding contract with the Life Esidimeni hospital and moved more than 1,300 patients to an “unstructured, unpredictable, substandard caring environment”, the report said.

“One person has died from a mental health-related illness. None of the 93 [others] have died from a mental illness,” the health ombudsman, Malegapuru Makgoba, told local media as the report was released.

“The Gauteng health department took patients from a licensed institution and handed them over to unlicensed facilities.”

Makgoba said the death toll was likely to rise as investigations continued into the scandal.

The report pointed towards the neglect that led to the deaths being caused by profit-seeking. The 27 healthcare centres “were mysteriously and poorly selected” and were “unable to distinguish between the highly specialised non-stop professional care requirements … and a business opportunity”, it said.

Gauteng’s premier David Makhura vowed to hold accountable all the responsible officials.

The opposition Democratic Alliance party expressed outrage over the findings, and accused the government of lying about the death toll when reports of the tragedy began to emerge.

“Criminal charges should also be laid against all implicated parties,” said the party’s provincial shadow health minister.

Psychiatrists attack "scandal" of child mental health spending

NHS bodies are spending as little as £2.01 per child on mental health care for young people, despite the big spike in anxiety, depression and other serious problems among under-18s.

Psychiatrists claim the small sums being spent by GP-led clinical commissioning groups (CCGs) in England constitute a national scandal at a time when youth self-harm and suicide are rising.

“It is a national scandal that opportunities to prevent mental illness from occurring in childhood are being missed because of unacceptably low investment,” said Prof Sir Simon Wessely, the president of the Royal College of Psychiatrists.

“Our analysis shows that in many parts of the country, the proportion of money that NHS clinical commissioning groups are planning to spend on the mental health of our children and young people is negligible,” he added.

Luton CCG is spending the least amount of money for every under-18 in its area this year – just £2.01 – according to research into CCGs’ spending plans undertaken by the royal college.

Other CCGs spending notably small sums on children and adolescent mental health services (CAMHS) include the London boroughs of Enfield (£2.33), Tower Hamlets (£4.45) and Ealing (£6.39) and North Somerset (£5.89).

In contrast, Birmingham South and Central CCG has allocated the most for such services of any of England’s 209 CCGs – £135.85 – while the figure is £131.37 in Corby CCG and £126.76 in South Tyneside.

Ten CCGs are spending less than £10 a head this year treating the growing number of psychological and psychiatric illnesses which are occurring in under-18s. Twenty-five will spend £25 or less per head. Experts say that exam stress, body image issues, the negative effects of social media and pressure to succeed are among the key factors behind the increase in mental illness.

“The tiny sums being spent in some areas of the country are disgracefully low, particularly in light of sharply rising need,” said Luciana Berger, the MP and former shadow mental health minister who is now the president of the Labour campaign for mental health.

“This is yet further evidence of the gap between Jeremy Hunt’s continued promises for increased funding and the reality on the ground for our young people’s mental health services.”

The Guardian disclosed last month that NHS mental health services were treating 234,000 children and young people in England every month, and that the real figure was much higher because 40% of mental health trusts had not yet said how many patients under 18 they had.

Claire Murdoch, NHS England’s national director for mental health, said it was putting more money than ever before into CAMHS services.

“Improving mental health for children and young people is an absolute priority for the NHS, and councils, third sector organisations, schools and social care all have a role to play in this. The additional £1.4bn invested last year is helping us to make progress on our ambition to treat 70,000 more children and young people each year by 2021,” said Murdoch.

CCGs increased their spending on mental health overall by 8.4% during 2015-16, she added, but acknowledged that there was “variation around the country” in the exact sums invested.

But research earlier this week by the Education Policy Institute said much of the extra £250m a year which the coalition government pledged to put into CAMHS care during each of the five years of this parliament was not reaching the frontline. Specialist children’s psychiatrists in many areas agree.

Berger said that underfunding CAMHS care was “a false economy” because 75% of adults who developed mental health problems first exhibited symptoms in childhood, and problems left unaddressed cost the NHS more.

The NSPCC said the wide variation in the amounts put into CAMHS care reflected a “postcode lottery”.

A spokesman for the charity said: “It is a sad state of affairs if vulnerable children and young people are subjected to a postcode lottery when it comes to getting mental health support.

“It’s really concerning that some areas of the country will only spend a fraction on mental health services for children and young people compared to what others will spend elsewhere. It should go without saying that children and young people should be able to get proper support for mental health conditions regardless of where they live.”

Government inaction on obesity is a scandal | Letters

Obesity is the biggest public health crisis facing the UK and it is crucial that we tackle this issue directly (Ministers accused of backtracking on childhood obesity plan, 31 October). It is staggering that this government is choosing to ignore the problem. Is it acceptable that a third of children aged two to 15 are overweight or obese, putting them in danger of a variety of long-term health problems? The government had a fantastic opportunity to introduce tangible measures that could have initiated a culture change, but this strategy barely touched the surface.

In 2013, the Academy of Medical Royal Colleges produced a report, Measuring Up, that identified actions to tackle obesity, including banning junk food advertising before 9pm. We hear that restrictions on junk food advertising were originally included in the government strategy, so why were these and other measures supported by medical experts, removed?

We know what the issues are and what needs to be done to make a difference, so what is stopping our politicians from getting things moving? If we don’t act soon, the number of people living with illnesses caused by obesity will increase significantly and the financial burden on the NHS will continue to rise. We can’t afford to wait any longer to confront this problem head on.
Dr Sheena Bedi
Chief executive, ABL Health

• You rightly focused on nutrition, but the issue of physical activity is also crucial. I’m told by teachers that secondary schools are ceasing teaching PE altogether and using the time in the pursuit of academic exam results. There is no requirement to teach PE and what schools should cover is also not prescribed. If PE is not taught, it will be a disaster not just for kids’ health but their development, confidence, social skills and general education.

Failure to protect women from FGM is "ongoing national scandal", MPs say

The failure of successive governments to protect vulnerable girls from female genital mutilation is an “ongoing national scandal”, according to a report by MPs published on Thursday.

The cross-celebration Commons property affairs decide on committee calls for schools to get rid of funding if their headteachers do not read through advice issued earlier this yr following a Guardian campaign.

Soon after hearing from victims, wellness and social employees, police and attorneys the MPs also explained there was a situation for emulating the French model by checking up regularly on at-danger youngsters, but they stopped quick of endorsing necessary gynaecological checks. It is estimated that 24,000 girls under the age of 15 are at danger of female genital mutilation in the United kingdom.

Keith Vaz, the committee’s Labour chairman, said victims had been badly let down. “FGM is an ongoing national scandal which is probably to have resulted in the preventable mutilation of thousands of ladies to whom the state owed a duty of care,” he said.

“Successive governments, politicians, the police, well being, education and social care sectors need to all share responsibility for the failure in recent many years to react adequately to the developing prevalence of FGM in the United kingdom.”

The MPs welcomed the determination taken by the education secretary, Michael Gove, to publish to all schools in England and Wales warning them of the dangers of FGM, following a Guardian-backed petition attracted more than 230,000 signatures this 12 months.

Even so, the committee members mentioned the Division for Training had to do a lot more to make sure teachers had been informed and need to send the guidance out once again.

The report stated: “To guarantee that the guidance has been looked at, the Division for Education should link the receipt of a proportion of school funding that relates to social training and youngster safety to the electronic notification that the guidance has been viewed.

“We further advocate that headteachers and little one safety officers, where they have not presently done so, undergo compulsory safeguarding education which specifically bargains with FGM.” The MPs also say:

• Any little one seen as becoming at danger of female genital mutilation must have that view regularly recorded in the child’s individual wellness record. Safety orders need to be introduced for individuals at danger…, as well as provisions to make certain women living in the Uk but with no British passports do not slip through the net.

• Much better solutions are essential for survivors, which includes refuges for ladies at danger of female genital mutilation.

• Failure to report female genital mutilation ought to be produced a criminal offence if reporting of the practice does not improve in the subsequent 12 months.

Keir Starmer, the former director of public prosecutions, mentioned the report was a considerable milestone for the campaign to end FGM. “The proposals are far-reaching and will want to be worked by means of, but the recognition that all companies have a duty for FGM prevention and reporting is considerable. Even though prosecutions right after the occasion send a quite crucial message, schooling and instruction to avert FGM in the first location is greater.”

Starmer extra that even though proposals to support victims better, such as anonymity, have been welcome “in the end, the greatest prospect of success in enforcing the law lies with proactive policing and intelligence-led instances”.

Campaigners largely welcomed the pick committee report but voiced aggravation with a lack of some specifics. Lisa Zimmermann, of Integrate Bristol, welcomed the proposal to make training of teachers compulsory. “We are thrilled by the report, education is definitely crucial in placing a quit to FGM – we just hope that the government now acts on these suggestions,” she said.

Efua Dorkenoo, director of the Finish FGM campaign at Equality Now and a long-time campaigner, stated the report was “extremely good” but could have gone additional.”To have the endorsement of the house affairs committee on this is really important and it is the most significant acknowledgement of the issue we have had so far from parliament,” she mentioned. “I would have liked for them to make the failure of pros to report FGM a crime, because there is nevertheless resistance between some specialists – the government wants to make the decision for them.”

Sarah McCulloch, who runs the grassroots anti-FGM charity ACCMUK, said: “The report is wonderful but for me it has failed to give recommendations as to what experts need to do or make the government supply funding for neighborhood groups to tackle the practice on the ground.”

Dr Peter Carter, standard secretary of the Royal School of Nursing, said the RCN was currently in the procedure of updating its personal guidance on FGM. “The report correctly identifies the need for education, advice and assistance to help health care workers talk about this issue with sufferers, determine these who are at chance of FGM, and get proper action,” he mentioned. “It is essential that this momentum is stored up, with improved coaching and help for employees, enhanced awareness, and prosecutions, to send the message that FGM has no place in the United kingdom, or any other society.”

The scandal of widespread mental illnesses left untreated | Daniel and Jason Freeman

Envision you are the campaigns manager of a political get together. You are conscious of a public well being crisis that, at any 1 time, has an effect on a third of the population, lowers existence expectancy as drastically as smoking, is much more disabling than angina, asthma, or diabetes, and minimizes GDP by around 4% every single yr. You know this crisis can be considerably – and cheaply – alleviated. Wouldn’t you make the issue a central theme in your election campaign?

Not in the United kingdom, it would seem. The crisis in question is a single of mental overall health, however what our politicians propose to do about it stays unclear. If they are in need to have of assist, they could usually start off with a mindful reading of a new book by the economist Richard Layard and clinical psychologist David Clark.

At the heart of Thrive: The Electrical power of Evidence-Based mostly Psychological Therapies is a bewildering conundrum. Mental illness is very widespread: one in 3 individuals will expertise a problem at some level throughout their lifetime in any one year, 19% of us will endure from an nervousness disorder, 13% from a substance problem, and seven% from depression. If private misery on this scale isn’t ample to move politicians to action, an yearly price to the exchequer of about £28bn (not counting NHS expenses) may be expected to do the trick. Nevertheless most individuals get no health-related aid for their circumstances, and of the minority that do get therapy, really few get the most successful form.

This is a scandal that tells us significantly about the lack of importance our society areas on mental health. Had been these individuals suffering from a significant and continual physical ailment, the story would surely be extremely various. Would we tolerate a circumstance in which the majority of individuals suffering from diabetes, heart illness, or arthritis had been left to fend for themselves, or asked to make do with inferior therapies?

The injustice is compounded by the reality that we now have effective treatments for a very wide selection of psychological problems. A lot of of these remedies – verified by dozens of randomised controlled trials – are variants of cognitive behavioural treatment, even though other therapies this kind of as counselling are powerful for specified problems.

CBT is at times criticised as an overly simplistic, when-dimension-fits-all method. Layard and Clark remind us that when carried out properly CBT is far much more nuanced. For each and every difficulty, clinicians build and test a certain theoretical model of symptoms and leads to and on this basis create a targeted therapy technique. The aim is not to produce a blithely complacent Stepford population, but to help individuals obtain meaningful and optimistic modify in their lives.

CBT is not simply successful, it is also comparatively low cost – undoubtedly when in contrast to the spiralling costs of medicines such as antidepressants. Opponents imply that this is what makes CBT appealing to policymakers, as if value have been an index of effectiveness. They overlook the reality that if it were genuinely so appealing to politicians it would be offered to all who require it, just like hypertension medication or insulin. Nevertheless most men and women are utterly unaware of the existence of these new psychological therapies.

Layard and Clark are not, it must be stated, disinterested observers. But they undoubtedly know, from extensive first-hand knowledge, whereof they speak. Clark is arguably the country’s most eminent clinical psychologist and a pioneer in the treatment of anxiety. Layard, a life peer and a leading economist at the London School of Economics, has led the get in touch with for happiness to be regarded a crucial criterion of national good results. Together, they developed the notion of the Improving Access to Psychological Therapies (IAPT) scheme, which was launched in 2007 with the aim of education 6,000 new therapists and supplying help for the 15% of individuals who produce typical mental wellness issues every single yr.

IAPT has helped 1000′s of people, but it’s as well tiny-scale. The dilemma lies with funding bodies – healthcare commissioners in the Uk or insurers in the US – who see psychological therapy as an effortless area to minimize. “Many will only pay out for 6 sessions of treatment,” say Layard and Clark, “as if it might be all right to authorise half a heart operation.”

Antidepressant medicine, on the other hand, is doled out by physicians on an industrial scale. But of program pharmaceutical businesses are immensely wealthy and extremely energetic lobbyists. Advocates for psychological therapies can’t compete, even though investigation exhibits that this is the variety of remedy that people would desire to obtain.

CBT, as Layard and Clark acknowledge, does not perform for absolutely everyone. And it doesn’t suggest that we really don’t want to tackle the triggers of psychological illness, this kind of as poverty, tension, and lack of social support. But it’s time we got serious about tackling psychological problems, ramping up research and providing folks with the remedies that have been proven to function.

Just five% of the United kingdom wellness study budget goes on mental well being. IAPT is run on a reasonably paltry £340m per 12 months. Can we afford to finance this expansion? That’s a query one particular could hardly picture being asked in relation to, say, heart illness. Furthermore, Layard and Clark argue that it would price us nothing in net terms, since of financial savings in welfare payments (such as sick shell out) and bodily healthcare expenses (mental and bodily wellness issues are usually interlinked, which means that we can assume a 20% reduction in bodily healthcare costs as a direct outcome of psychological therapy).

We require to increase radically the IAPT scheme: growing the quantity of therapists boosting the percentage of folks with problems who can acquire therapy minimizing waiting lists covering all geographical regions offering therapy to kids as well as adults including individuals with chronic bodily well being circumstances and delivering assist for these with significantly less typical, but typically really disabling, mental overall health circumstances this kind of as schizophrenia.

Failing to do so, as Layard and Clark conclude, would be “sheer discrimination towards people who are mentally unwell, and an abuse of human rights … Potential generations will be astonished at how blind we were. They will also be amazed that we were so cruel.”

Ignorance of care high quality is "true scandal" facing NHS, warns overall health chief

“The true scandal in our wellness service is ignorance,” he explained.

“If you consider about cancer, I couldn’t tell you how a lot of individuals are receiving chemotherapy,” he stated. “Not even how several people are obtaining remedy, allow alone at what benefit.”

The ideas for care.data – a database of GP records – have been delayed until up coming yr, and will stick to pilot schemes in a number of regional regions.

Mr Kelsey explained he hoped that a number of new safeguards would offer the public with assurances that the information would only be used to improve patient overall health care.

He explained: “We are fully flying blind. The NHS – such an essential, extremely valued public religion truly – is offering a service that on the entire has no thought of its high quality.”

The NHS official explained his own family’s attempts to blow the whistle on poor overall health care had convinced him of the need to have to have far more transparent information published about companies.

He mentioned his mother had been “hounded out” as a GP after attempting to warn that a hospital advisor appeared to be repeatedly missing situations of breast cancer.

Published information comparing NHS providers could reveal large death costs and poor companies, he explained, alternatively of leaving it to individuals to warn of suspicions over poor care.

He stated the NHS repeatedly taken care of whistle-blowers appallingly, ending careers.

“My experience of whistle-blowers has universally been tragic and there is no excellent factors that come to people who break the bounds of their organisation and inform the reality. Its virtually always a catastrophic emotional experience for people individuals,” he mentioned.

Mr Kelsey stated the strategies for the giant GP database were “the greatest data revolution” any public support had embarked on, even though admitting that numerous questioned him as to no matter whether the scenario had turned into “a vehicle crash”.

He stated: “We chose to delay the programme earlier this yr soon after a multiplicity of hugely-justified concerns about the way it was getting rolled out.”

“We paused it due to the fact there was this tremendous outcry, and uncertainty about what was happening.”

He mentioned officials had now introduced important safeguards in response to many of the concerns and explained the NHS need to be “celebrating” the fact that the backlash had led to a large public conversation about the plans.

Mr Kelsey dealt immediately with a amount of worries raised, in particular an investigation by The Every day Telegraph, which found in February that NHS hospital information had previously been sold for insurance coverage purposes.

He stated: “Data of this kind can only ever be utilised now for wellness benefit – so we can rule out the emerging concerns that perhaps men and women would use this for insurance or for junk mail. That is not legally permitted any longer.”

New legal action in thalidomide scandal

Much more than 10,000 infants were affected globally, of whom a lot more than half died. About 460 recognised thalidomide victims still live in Britain.

Slater &amp Gordon solicitors has issued new proceedings against Grünenthal, the German pharmaceutical firm which created thalidomide, and Diageo Scotland, which in the late 1990s acquired Distillers Co (Biochemicals), the company which distributed the drug in Britain.

Fraser Whitehead, the claimants’ solicitor, explained: “It is a tragedy that folks with important bodily abnormalities brought on by thalidomide have had to reside with their troubles for much more than 50 years yet have by no means been compensated by the producer and distributor.

“Grünenthal and Distillers have usually claimed that the thalidomide disaster was an unavoidable tragedy and that they did almost everything anticipated of drug businesses at the time. Our study has demonstrated that is nonsense.

“Both firms knew their drug was leading to severe nerve damage and each had been explicitly warned of the chance thalidomide may possibly lead to severe malformations. Yet the drug was left on the market place for a lot of months afterwards.

“It is time these accountable are last but not least held to account for the mistakes of the past. Grünenthal, in distinct, has completed small make great the immense damage that it has done and it has to accept accountability for these impacted in the United kingdom.

“Thalidomide survivors in this nation have had their lives ruined by this drug nevertheless they have not obtained any justice from Grünenthal in spite of the organization becoming held to account in other components of the planet.”

Distillers agreed to pay compensation to recognised thalidomide victims in 1973 but the new legal situation claims at least eight individuals missed out on settlement offers due to health care criteria utilized to claims at the time.

Mr Davies, from Gwynedd, North Wales, was born with significant defects of each legs. His left leg ends in a stump below the knee and his correct leg has only two toes.

The married father-of-two said: “I’m very angry at the way I, and other folks like me, have been handled over the many years. I have by no means actually been provided any assistance however my disabilities have meant my daily life has been quite difficult.

“I have to wear a prosthetic leg and suffer consistent pain in my legs, hips and back. I be concerned that the soreness will maintain obtaining worse and end me from doing my occupation.”

He additional: “I know I am luckier than numerous as I nevertheless have my arms but that does not mean I ought to have been ignored. It is a disgrace that so a lot of people have been denied justice in this way.

A Diageo spokeswoman explained: “We consider any matter relating to thalidomide injured folks very significantly.

“We have a extended track record of performing the proper point in our treatment of thalidomide injured individuals in the United kingdom as evidenced by our prolonged-standing connection with the Thalidomide Trust and the £60 million of funding we have supplied to the trust to make certain the prolonged-term care and compensation for these genuinely affected. As far as we are aware, no legal proceedings have presently been issued towards us with respect to this matter.”

Chairman of Mid-Staffs scandal inquiry speaks out

“I have realized from my son, and from other trainees, that we can find out a whole lot from the fresh eyes and idealism that medical professionals and nurses have when they start off out. It is so crucial to exploit that, and not to crush it.”

The barrister believes that a lot of of the worst failings in the NHS take place when clinical staff become powerless — are left “shrugging their shoulders” rather than demanding bad care.

It is 15 months because Mr Francis produced 290 recommendations to radically reform the health services in the wake of the Stafford Hospital scandal. The trust was criticised for causing the “suffering of hundreds of people” in its care amongst 2005 and 2008.

Since individuals recommendations had been created, the Government has ordered a host of adjustments — and Mr Francis has been appointed president of the charity the Sufferers Association, a position which was previously held by Claire Rayner, the agony aunt and former nurse.

In his 1st main interview because the Government responded to his inquiry, he says he is encouraged by most of its actions, which have included an overhaul of NHS regulation, potential ratings for hospitals and improvements to coaching and recruitment of employees.

Even so, Mr Francis expresses disappointment that ministers chose not to introduce his recommendation for a legal duty of candour to be placed on health specialists, which would have obliged workers to communicate up about patient dangers. “We do want to defend people by making confident they really feel risk-free to report factors to their employers,” he says. “I felt a statutory duty would have assisted that — as well usually those who raise concerns about things that go incorrect turn into unpopular with colleagues and they need to have some form of safety.”

He is also concerned about “complacency” in the well being service, with too fantastic a tolerance of mistakes and failings in care which can prove catastrophic. “The huge majority of these receiving care in an NHS hospital get perfectly acceptable care,” Mr Francis says. “The difficulties is it’s no use being happy or complacent — if we ran our airline industry on the identical basis planes would be falling out of the sky all the time.

“We’ve just got to modify the frame of mind that due to the fact it’s presented by the state, it is all appropriate for a number of individuals to be taken care of badly — effectively, it is not. Airlines would go out of enterprise really rapidly if they worked that way.”

Mr Francis believes that the culture of the NHS is altering, with “a greater atmosphere of openness and candour”.

He credits Jeremy Hunt, the Overall health Secretary, for a “refreshing change” of strategy in standing up for individuals, suggesting that his predecessors — from both events — tended as an alternative to act “as a spokesman for the NHS”. Mr Francis suggests that deference to medical professionals — “allowing them a God-like status” — and pride in the NHS have stifled political debate about its failings.

Given that March, hospital trusts have been obliged to publish particulars of all “never events” – safety blunders so basic and significant they can’t be justified.

The public wants nonetheless far more details, he believes, in order to consider an informed see about exactly where to go for remedy and for a meaningful debate about the potential of NHS solutions.

He also raises issues that essential areas of care, this kind of as that of the elderly, are too frequently starved of funds they need, as society is less very likely to shout about it.

“I think one particular of the factors that the elderly don’t get the priority they must — and the identical possibly applies to mental wellness — is that there’s been a reduction in social knowing about the responsibilities we have in the direction of these folks in society,” he says.

He believes also many hospitals use economic difficulties as an excuse for poor care. “If you can’t afford to seem soon after your previous people or your children safely that is just unacceptable,” he says, “and I really do not think when it comes down to it that lack of income is a justification.”

Mr Francis grew to become president of the Patients Association final November. It is a part the charity has not filled since the death of Mrs Rayner four years ago, as it was established to discover the appropriate man or woman. Given that taking the place, he says he has grow to be more and more concerned by the failure of the NHS to deal pretty and compassionately with people who complain of bad care. The charity is “deluged” by correspondence from these whose complaints have fallen on deaf ears, he says. 1000′s of individuals and bereaved relatives make get in touch with.

“The letters I get are from individuals who are misplaced and they have nowhere to go,” he says. “It’s extraordinary how lonely individuals can be — it damages them. Typically these are men and women who have lost a relative so they are grieving.”

The barrister says he comes from a different mould to his predecessor but would like to perform a element in modifying the culture of the NHS, to place the patient’s voice centre stage. “I’m not an agony aunt,” he says, “and I am not here to put into action the adjustments that I recommended, but when it comes to speaking up for patients, the Sufferers Association is the only game in town. If I can help to make some of the adjustments a practical reality, then I would like to do what I can.”

In an interview with The Telegraph, Robert Francis QC mentioned the public had been given a falsely good impression about the good quality of care getting presented in a lot of of the country’s hospitals.

The barrister mentioned the NHS necessary to make radical changes to meet the demands of the public – as an alternative of merely blaming individuals for “crowding out” Accident &amp Emergency (A&ampE) since they did not know the place else to turn.

Mr Francis mentioned that for too lengthy, those in charge of the health service had become “complacent” about the care meted out to individuals, believing it was realistic for some individuals to be badly failed, as long as the bulk have been not harmed.

Uk child death fee: a scandal with a clear hyperlink to poverty

Babies in a maternity ward

The death of a little one is usually a tragedy. The avoidable deaths of thousands of children beneath five many years old every single yr in the Uk is a scandal, in accordance to the Royal College of Paediatrics and Child Overall health (RCPCH) and the Nationwide Children’s Bureau, who say that inequalities in our society which depart numerous households in poverty and deprivation are to blame.

Their views, expressed in a report this week, Why Kids Die, put flesh on stark new statistics published on Friday from the most authoritative variety-crunchers on overall health in the globe, the Institute of Health Metrics and Evaluation in Seattle, portion of Washington University. In their paper, published in the Lancet, they demonstrate that young children under 5 in the United kingdom are much more very likely to die than in any other western European country except Malta. Almost five babies of each 1,000 born in the Uk will not live to their fifth birthday.

The information is broken down by daily life phases. The United kingdom does especially badly in the very first 6 days of life, in one month to a yr and – worst of all – in ages one particular to four. The factors are slightly distinct, but there is one particular outstanding hyperlink issue: deaths rise with socio-financial deprivation.

In 2012, three,000 died below the age of one. People babies tend to be premature and of lower birthweight. Their mothers could be youthful and still growing themselves, they may possibly be smokers or they might not eat effectively – the RCPCH report says the growing amount of people accessing meals banking institutions is a concern.

Once previous their initial birthday, the principal causes of death are diverse. Accidents and injuries occur. Young children drown in ponds or are knocked down by vehicles and lorries.

But once more, it is the children from the poorest communities who are most most likely to die. Their streets are not so risk-free, perhaps their families can not afford excellent childcare, there is nowhere safe to perform.

In this older age group, in accordance to Dr Richard Horton, editor of the Lancet, there is one more important issue. Ailments like cancer are uncommon in children but can be fatal and they are hard for a GP to spot. In our anxiety to defend the NHS in latest many years, he says, we have omitted to place young children at the centre of our issues. “Need to we not be pondering of a model of care in which we have paediatricians sitting side by side with GPs?” he asks. We should feel of taking the specialist to the kid as an alternative of the child to specialist.

The identical lesson have to be realized in other areas too. “Hospitals are not the potential,” he says.

Healthcare reorganisation is naturally a extended way off, but the target on poorer communities is urgent, in accordance to all people who know about little one well being and child deaths.

Sir Michael Marmot, who was concerned in the RCPCH report, has been speaking for many years now about the social inequalities that underlie bad wellness. This is a British medical doctor who has turn out to be the world’s guru on the situation. But although he is taking the message all around the globe, his personal country appears to be failing its youngsters.

The toxic reality about air pollution: a lethal scandal of British inaction

Smog In London

The City of London shrouded in smog. ‘Slowly, it is dawning on individuals that the risks from air pollution are far greater than previously imagined or understood.’ Photograph: Barcroft Media

Blame the Sahara desert for the existing air pollution. Blame Europe. Blame climate change – or even the spring sunshine, or the hole in the ozone layer. But if you are in government please will not mention the reality that the toxic air significantly of Britain has been breathing is primarily of our very own creating.

Never appear on the front page of Defra’s web site for an explanation of its composition, or assume ministers to admit this is a public well being emergency. And undoubtedly do not anticipate regional or central government to consider action, such as lowering auto numbers in the streets or closing down factories. That is what the Chinese and French governments do when the air in their cities is unbreathable and their individuals are choking. Not us.

What we are not becoming advised is that the milky, hazy skies shrouding southern Britain are the result of small particulates, or aerosols, that scatter sunlight and come from our personal targeted traffic, energy stations, farming, construction sites, central heating boilers and business. Our personal undesirable air is mixing with pollution drifting in from mainland Europe and a bit of Saharan dust, picked up by uncommon weather problems that have trapped the foul combine in excess of a wide spot. It will get till Thursday at the earliest before westerly winds start to disperse it, the Met office says.

But the final results of this will be witnessed over the coming months as hospitals report enhanced admissions from people struggling respiratory conditions, strokes, heart attacks and worsening lung conditions. Final yr, according to the government’s personal figures, more than 29,000 folks died prematurely from air pollution in Britain, with 4,300 in London alone.

But premature death is just the tip of the iceberg. In the brief phrase, polluted air leads to irritation to the eyes, nose and throat, headaches, nausea, bronchitis and pneumonia and above a longer period it results in heart attacks, lung illnesses, cancers, and injury to the brain, nerves, liver, and kidneys. We now know that young children residing near busy roads can present lowered lung capacity by the age of five, and that the bad are most impacted as they reside in the most polluted areas.

Gradually, it is dawning on individuals that the dangers from air pollution are far better than previously believed or understood. Last week, the incredibly cautious Globe Overall health Organisation revised its figures and reported that almost a single in eight of the world’s deaths – a lot more than seven million individuals a yr – are now the direct consequence of air pollution, and that for every person who dies, there are a lot of more whose overall health is impaired long term. Put bluntly: every single 12 months air pollution kills much more men and women than Aids, smoking, road accidents and diabetes mixed, and is the world’s single greatest environmental well being danger.

Final week, even as Paris banned some visitors to reduce air pollution, London played down the dilemma, even even though our air was just as polluted. London’s mayor, Boris Johnson, has many instances warned the vulnerable to avoid outside exercise, but he is accused of performing virtually nothing at all to reduce the causes of pollution. As the London assembly member Jenny Jones, Close friends of the Earth and other folks point out, Europe’s most polluted key city has no emergency plans to deal with air pollution and no powers even to restrict targeted traffic throughout intervals of smog.

Central government, too, has experimented with to wash its hands of the problem despite the fact that it is aware of that air pollution is costing the NHS billions of pounds a year, and that the supreme court has ruled that the state has a legal duty to protect us from it. Final month the EU finally, right after 15 years of warnings, extensions and postponements, launched legal proceedings against Britain for failing to reduce “excessive” levels of air pollution. The government now faces fines of £300m a 12 months, but we can assume it to try everything to lessen pollution – bar restricting targeted traffic.

The require to clear the air is just as wonderful nowadays as in the 1950s when peasouper smogs killed thousands. In 1956 a brave government passed the 1st Clean Air Act that banned coal burning and reduced pollution spectacularly. Today we have the laws from Europe that would do the exact same, but no government looks ready to put into action them.

Psychological well being units "are heading for a Mid Staffs scandal", warns senior psychiatrist

Sue Bailey

Sue Bailey said mental overall health providers are approaching tipping stage. Photograph: Anna Gordon

Britain’s senior psychiatrist has warned that the mental overall health sector is heading towards its personal Mid Staffs scandal, as it emerged that NHS patients are currently being handled in a hospital in which workers have been lately accused of utilizing injections and seclusion as threats.

Professor Sue Bailey, president of the Royal School of Psychiatrists, mentioned she was increasingly concerned that psychological services were approaching a “tipping stage” with funding becoming lower regardless of a record 50,000 uses of the Mental Overall health Act to detain individuals in hospital for assessment or therapy in 2012-13.

Regardless of the government’s claims that mental health would be offered parity with bodily health, Bailey stated money that it was due to be given had been redirected to deal with the repercussions of the Francis inquiry into the scandal of the needless suffering, and attainable needless deaths, of hundreds of men and women at Stafford hospital.

Bailey warned that the mental overall health sector could only “cut its cloth” so far. She mentioned: “I do truly feel really strongly that if we keep constantly becoming the last in the line there will come a tipping stage and we will end up with our very own [scandal and subsequent inquiry]. We are under increasing stress. Carers will tell you that. Mental overall health practitioners who really feel it is risk-free to talk about these issues will tell you that – but whistleblowing is nonetheless not simple.”

Bailey spoke out as it emerged that a juvenile patient at a private mental overall health secure unit in Woking, Surrey, which has a chequered historical past of overall performance, had told Care Top quality Commission (CQC) inspectors in December that seclusion and intramuscular injections were regularly employed as threats by personnel. During a go to to the hospital, owned by the personal Alpha Group, inspectors also identified that a younger particular person who had banged his head in seclusion was left lying on the floor for 15 minutes getting sick.

Inspectors could locate no information of healthcare assist being provided to a youthful man or woman in seclusion who was mentioned to have signs of “rolling eyes and losing balance” for at least thirty minutes.

The hospital, which was instructed in January to show proof of improvement within weeks, had already been told to urgently improve its functionality following two prior visits. In a prior report, inspectors recorded that “one particular female adolescent patient had been restrained by 9 members of staff, one particular of whom was a male” since she refused to take away her underwear.

Alpha Group, which has two even more hospitals in Bury, Higher Manchester, and Sheffield, is owned by the controversial Liberal Democrat donor Sudhir Choudhrie, who was named by India’s Central Bureau of Investigation as a single of 23 “unscrupulous persons” in 2012 and is accused of currently being an arms dealer, a claim which he denies.

Bailey said that exactly where failings were emerging they were a symptom of a program below strain. She explained: “Just like the rest of medicine, items can go wrong. We know the factors that make issues go wrong. And we know one of the things that created Mid Staffs go wrong was finance driving the complete kit and caboodle. We’re worried.”

In December a freedom of data request revealed that mental wellness trust budgets for 2013-14 shrank by two.three% in genuine terms from 2011-12. 10 out of 13 trusts that supplied forecast budgets for 2014-15 have been projecting additional cuts.

On Friday the CQC announced that Alpha hospital in Woking was now compliant with the “needed requirements” but that the regulator would proceed to keep track of the support. NHS England confirmed that NHS sufferers would proceed to be referred there. It stated that there had been no block on NHS referrals given that issues initial emerged at the hospital eight months in the past, regardless of the scathing findings above a series of CQC inspections.

Surrey has no other secure hospital in which juvenile mental well being individuals can be inpatients.

Victoria Bleazard, Associate Director of Campaigns at Rethink Psychological Illness stated she was concerned that the lack of different amenities in the region left vulnerable mentally ill children with no other alternative but the Alpha hospital.

She mentioned: “The government says that individuals must be able to decide on exactly where they get treatment for psychological illness, in the same way that folks with physical well being problems can. But when there is this kind of an acute shortage of hospital beds, the thought that they will be ready to pick where they receive therapy is completely unrealistic. For many men and women, receiving remedy at all is a struggle in itself.”

Alpha Hospitals explained: “We quite considerably value suggestions from the CQC. Following its inspection of 1 of our wards in November and December 2013, we worked closely with NHS England and CQC to create and implement an action plan. We have given that been inspected by the CQC and obtained optimistic feedback.”

Abortion scandal: MPs urge Jeremy Hunt to act on Crisis Pregnancy Centres

His sentiments are echoed by three other MPs nonetheless, the Well being Secretary has so far refused to comment, regardless of repeat attempts by The Telegraph.

The report, compiled by Brook, a nationwide sexual well being charity, discovered that a additional 36 (on prime of the two previously exposed by The Telegraph) of 135 CPCs distributed misleading information or attempted to emotionally manipulate vulnerable women who had entered their centres for tips. These centres are dotted across the Uk and can be discovered in each key city, like Manchester, Birmingham, Oxford and Edinburgh.

A single counsellor informed an undercover researcher that terminations can lead to ladies committing criminal acts, the report claimed.

Two other CPCs presented females ultrasound scans, with promotional materials that ‘make it clear that scans are carried out with the sole intention of persuading men and women not to have abortions’, in accordance to Brook.

The disclosures will anger advocates of impartial abortion advice, who argue that the centres are bankrolled by ‘pro-life’ religious organisations.

Dr Sarah Wollaston, a former GP who is now the Conservative MP for Totnes and a member of the Health Choose Committee, reiterated her calls nowadays for Mr Hunt to overview the abortion counselling providers: “There has to be transparency about who is funding these organisations and whether or not they are anti-abortion.

“If a ‘clinic’ is providing healthcare guidance it must come beneath the remit of the Care Quality Commission which ought to then have the powers to shut it if it is giving out entirely false info.

“It is time for a standard review of these counselling services.”

The Brook report, which builds on The Telegraph’s findings, also discovered that at least four CPCs with religious affiliations had infiltrated GP practices and hospitals.

Other CPCs, none of which are regulated, were receiving referrals from local GPs and hospitals, the report explained.

Issues have been raised in latest many years more than the objectivity of guidance provided ladies considering termination. Anti-abortion campaigners think that women need to go to an independent centre ahead of undergoing the process because they say there is a likely conflict of curiosity in ladies acquiring counselling from an organisation that also carries out terminations, such as BPAS.

Brook researchers were also provided pictures of foetal growth, medically unsound tips and counselled by advisers with ‘a clear bias in direction of adoption’.

In 2011, a bill launched in the Commons to stop abortion suppliers from pre-abortion counselling was defeated. The government also scrapped ideas for a consultation on whether women need to be necessary to undergo independent counselling prior to an abortion.

Nadine Dorries, MP for Mid Bedfordshire, who proposed amendments to the Overall health and Social Care Bill 2011, mentioned in light of the Telegraph’s investigation:: “Jeremy Hunt demands to create how abortion counselling must appear.

“We need to go to a professional organisation like the British Association for Counselling and Psychotherapy and inquire them to build, on behalf of the government, a non-directional, agenda-totally free, abortion provision counselling model that we can then adopt into the guidelines.”

The Brook report concluded that the top quality of services supplied by CPCs was ‘extremely variable’ with significant numbers of CPCs spreading ‘myths’ about abortion via web sites, leaflets and counselling sessions.

Simon Blake OBE, Chief Executive of Brook, stated: “It is merely immoral and unacceptable to masquerade as a support that assists clients make decisions through exploring selections, when the reality is – as this report primarily based on younger women’s experiences displays – at some CPCs you will get a mixture of misinformation, coercion and fright tactics.”

Luciana Berger MP, Labour’s shadow public well being minister, said:

“Anyone studying these comments produced to ladies by people purporting to be counsellors will be disturbed.

“In light of this emerging proof on ladies obtaining incorrect details, we have to look once again at the lack of regulation in this region.

“Abortion counsellors need to be neutral. It are not able to be correct that they provide advice to women not based mostly on evidence.”

A spokesperson for the Division of Wellness stated: “The provision of non-judgemental counselling about abortion is obviously essential. It is for NHS suppliers to make certain that the companies they advocate meet this want.”
