Pets Help Kids Build Immunity Even Before They’re Born, Study Finds

If you’ve been considering adopting a dog or a cat, this new study might help you make up your mind: According to research published in Microbiome, children born into homes with pets have higher levels of gut microbes that protect against allergies and obesity. Researchers say that indirect exposure to furry friends is especially beneficial to babies in their first three months of life, and even while they’re still in the womb.

This period appears to be a “critical window of time when gut immunity and microbes co-develop,” says lead author Anita Kozyrskyj, PhD, a pediatric epidemiologist at the University of Alberta. Disruptions to this development process—for example, the introduction of foreign bacteria—can change immunity and have implications for long-term health, she adds.

Kozyrskyj’s study compared fecal samples from 746 Canadian infants. Some of the infants were from families with pets (70% of which were dogs), some were from families who had pets during their pregnancy but not once the babies were born, and some had no recent pets.

The researchers found that babies from households with pets had twice the amount of two specific bacteria, Ruminococcus and Oscillospira, that have been linked with reduced risk for childhood allergies (such as dermatitis and asthma) and obesity, respectively.

Even when pets had been given up for adoption before the babies were born, this benefit was still seen. This shows that pet exposure affected the babies’ microbiomes indirectly, with influential bacteria transferring from the animal to the mother to the unborn child.

This bacterial exchange occurred regardless of the type of birth (vaginal versus C-section), regardless of whether the babies were breastfed, and regardless of whether antibiotics were used during birth. This is important, say the authors, as previous research has shown that C-sections, antibiotics, and not breastfeeding can all negatively impact babies’ immunity.

The presence of pets in the house during a woman’s pregnancy was also associated with a reduced likelihood that she would transmit group B strep bacteria—which can colonize in the vagina and cause pneumonia in newborns—to her baby during birth.

The study builds on decades of research showing that exposure to dirt and bacteria can help children build up immunity, and that kids who grow up with pets have lower rates of asthma and allergies, says Kozyrskyj. (This effect is strongest with dogs but is true with cats, as well.) “Pets have always been our best friends and they appear to be good for our babies too,” she says.

But further studies are needed to provide definitive evidence that a “dog intervention” during pregnancy can actually prevent allergies. “Our next step is to pursue the ‘does it matter’ question, to determine the allergy and obesity outcomes of studied infants at ages 1, 3, and 5,” Kozyrskyj says.

And of course, allergies and obesity aren’t products of the environment alone; genetics also play a role. It also makes sense that kids with pets have natural protection against allergies, since parents with allergies are less likely to have dogs and cats in the first place.

But the study’s results were adjusted to account for whether the mothers had asthma or allergies, and still held true. Kozyrskyj says her team’s findings “are independent of these two possible explanations.”

It’s not far-fetched to think that pet exposure during pregnancy could one day be an official recommendation from doctors, Kozyrskyj says. It’s even possible that scientists could develop a “dog in a pill” supplement, she adds, for mothers to take as a preventative measure against allergies and asthma.

More research is needed for these scenarios to actually happen, however. For now, families have have another reason (on top of plenty already!) to snuggle up to their fur babies—and that includes moms-to-be.

My vegan experiment: ‘Even in my maddest moments I never saw myself doing this’

The first thing I want to say is that this is the last thing I ever expected to write. Over the years, I have contemplated joining the police, moving to Canada, and taking up darts, but even in my maddest moments I never saw myself doing this. The need I feel to make this disclaimer crystal-clear, however, is something I’ve been puzzling over. The only purpose it can serve is my own vanity – which, I have been surprised to discover, is my single biggest worry about becoming a vegan.

It has only been six weeks, so it’s still early days, and I could be wrong about this. But I don’t think the worst thing about going vegan is saying goodbye to all the lovely things I used to eat. I really like roast lamb and scrambled eggs and ice-cream, but the truth is that you can get used to anything. My problem with veganism isn’t the diet, but the identity.

‘These days it’s the height of fashion and – if you’re a celebrity – practically compulsory.’ Jay Z and Beyoncé.

Veganism used to be so niche that I was in my 20s before I first met a vegan, but these days it’s the height of fashion and – if you’re a celebrity – practically compulsory. Famous vegans are ten a penny – Beyoncé, Jay Z, Jennifer Lopez, Brad Pitt. Unlike most celebrity lifestyle choices, any idiot with a Deliciously Ella cookbook can access the glamour of giving up animal products, which may explain why the number of vegans in Britain has almost quadrupled in the past decade to more than half a million. Living in London, one can’t fail to notice that even vegan fashion has assumed a newly confident air; the old passive-aggressive T-shirt slogans (“Vegan. Because My Body is Not a Graveyard”) have made way for knowing hipster humour (“All Hail the Kale”). On a street in Hackney where, last time I looked, crowds on the pavement meant a pub fight had spilled on to the kerb, hordes now queue patiently in the rain, like old Soviet Union shoppers, outside “the world’s first-ever” vegan fried chicken shop, the Temple of Seitan.

So I don’t worry about looking weird. I worry about becoming that person who thinks what they put in their mouth is important enough to merit a significant share of their time and energy every day. Who wants to be sourcing vegan mozzarella on holiday, forever scanning labels, making a nuisance of themselves at dinner parties? To me, it has always felt fundamentally precious, if not borderline narcissistic, to care that much about the purity of one’s digestive system.

That middle-age would force a reappraisal never even occurred to me. In my 20s, it was just about plausible to pass off a diet of Dime bars and Diet Coke as evidence of a life too fabulously busy for trivial distractions such as actual meals. By 45, however, it looks like self-abuse. After I was diagnosed with breast cancer 18 months ago, it altogether ceased to be an option and, for the first time in my life, I paid serious attention to the novelty of taking care of myself – not least because when surgeons, oncologists and nurses have done everything they can to save your life, there is a certain sense of obligation to do one’s bit, too. When nutrition is no longer a lifestyle choice but a medical necessity, eating food that might really kill you becomes patently idiotic – which is why I spent most of last year trying to adopt a healthier diet.

When nutrition isn’t a lifestyle choice but a medical necessity, eating food that might kill you is patently idiotic

The problem, as anyone who has ever tried knows, is that it’s impossible to know if you are doing it right. There is quite possibly greater global consensus on how to solve the Israel/Palestine conflict than on what we should eat, and the more you learn, the less you feel you know. Is a low-carb paleo diet the answer? Is meat OK as long as it is organic? Will five a day do the job? Or is anything less radical than a strict raw-food regime a waste of time? Willpower is hard enough to sustain, even when you know it’s worth it. The possibility that it might, in fact, be pointless was enough to derail every diet I tried.

In the end, I could find only one way to make sure everything you put in your mouth is a good idea. The simplicity of the solution was irresistible. The problem, of course, is that nothing else about going vegan is remotely simple. Was I really going to find time to start shopping in specialist wholefood stores and soaking mung beans? The vegans I knew all promised that after a month I would start to feel so fantastic that the palaver would seem a small price to pay. This may be true, but I worried that faffing about with legumes would defeat me long before the fabled herbivore high kicked in.

Was there, I wondered, an easier way to do it? Of course there was. I found a company in London called Detox Fit, a one-stop shop for lazy vegans, that offers a weekly delivery of gourmet meals. A sort of vegan ready meals on wheels, if you like. Run by a preposterously good-looking vegan couple, the company also provides an AA sponsor-style helpline to call should I find myself outside a kebab shop, having a wobble. If out and about on the high street and hungry, all I need to do is call and they will guide me to vegan options in the vicinity.

Unable to think of a reason not to, I signed up, and embarked on a three-month experiment with veganism. A finite commitment felt much more doable than a lifelong promise, but the very modesty of the challenge also made me anxious. With no excuse for falling off the wagon, what if I still did?

There are so many reasons to be vegan that it is hard to know which one would work best in a crisis of willpower. Health is mine, and the obvious one – but, in a reckless mood, I might decide not to care. Would thoughts of carbon emissions from cattle fortify me? Maybe the trick is to keep a mental archive of factory-farming photos, ready to deploy when necessary. Then again, what if world famine turns out to be the most compelling option? It might be an idea to memorise some eye-opening statistics about how many acres of grazing it takes to make a Big Mac, and how many people the same fields could feed if we planted crops instead. Adopting a belt-and-braces approach, I watched the appropriate documentary material online: Cowspiracy, Forks Over Knives and Earthlings.

Rather to my surprise, at the end of month one, I’ve had no need for either the Detox Fit hotline or my fortifying mental devices. Whatever had I been worrying about? The whole thing has been startlingly easy.

And then I was invited to a dinner party …

Why Donald Trump should play even more golf | Patrick Barkham

Scrolling through Twitter last night, I wondered why I felt so well. Not smug, just mystified by a mental and physical glow that made me strangely impervious to the toxic aspects of social media.

It had been an unmemorable weekend. I’d had a distressing argument with one daughter over her disinclination to go swimming; visited a garden centre without buying anything; spotted a marsh harrier; and played “the wheelbarrow game” in the garden, which involves carting one child in a wheelbarrow in pursuit of the others.

Then I read an interview with Florence Williams, author of The Nature Fix, who mentioned that we spend on average 5% of our day outdoors. I totted up my 15-hour Sunday: I’d spent exactly half outside.

We’re oblivious to one of our most obvious requirements. Homo sapiens is supremely adaptable, but westerners have lived predominantly inside for 10 generations at most. We need air, sky and some kind of engagement with the earth – but we’re bouncing off mental and physical walls.

There’s no intrinsic moral blessing to it. We may be shooting grouse, paintballing, cycling, gardening or climbing rocks, but a big portion of the pleasure is simply being outdoors.

There’s a furore about Donald Trump playing golf so often in the first weeks of his presidency. This might just be keeping him calm and sane. Play on, Mr President.

The west does not need the grinding physical labour that sent farm labourers to an early grave, but we must reimagine work and schooling to permit more time outdoors. We’d be wise to protect green space in cities and allow more of the solar-powered, pollution-fighting machines otherwise known as trees, which enhance our wellbeing more than we realise.

These are not new arguments. Octavia Hill, a co-founder of the National Trust, was banging on about “open-air sitting rooms” for the urban poor in the 1870s. But we now have neuroscience to prove what we overlook: we need nature far more than it needs us.

The nitrogen nightmare

stinging nettles

‘Thuggish’ species such as nettles thrive on excessive nitrogen and ammonia, smothering rarer wildflowers Photograph: Alamy

Detoxifying the air is our most immediately pressing environmental challenge – for obvious, self-interested reasons. Less obvious is the impact of air pollution on plants. Nitrogen and ammonia unleashed by burning fossil fuels, traffic and farm fertilisers are having a devastating impact on plants, according to the charity Plantlife. A staggering 90% of nature-rich habitats in England and Wales are suffering from excess nitrogen. In such conditions, a few “thuggish” species such as nettles thrive, smothering rarer wildflowers, fungi, lichen and the invertebrates that depend on them.

We need to talk about nitrogen, says Plantlife. We do. And somewhere in a parallel, enlightened world, politicians sport baseball caps embossed with the vote-winning slogan: Reduce nitrogen Deposition Now.

Notes on a very small island

The British Isles is an archipelago of about 6,000 islands but we lost a big one, Doggerland, 8,000 years ago. Our neolithic forebears would have been delighted by plans to rebuild a 2.5 sq mile island on Dogger Bank, in the North Sea. The old Doggerland was swamped by rising seas; Doggerland 2.0 will be devoted to renewable energy, helping to lessen the sea-level rise caused by global warming.

Sadly for romantics, this idealistic island is called the North Sea Wind Power Hub, and will be built by energy companies from Denmark, Germany and the Netherlands. This is our ancestral home. We must participate, if only to preserve the Doggerland brand. But I suppose Brexit means we’re a lonely set of small islands these days.

Soon we"ll be able to spot diseases like cancer before we even feel sick

The sooner a disease is diagnosed, the more likely it is to be well managed or cured. The challenge to finding a disease early is that most of us don’t seek treatment until we have symptoms, which means the disease has already progressed.

But breakthroughs in nanobiotechnology techniques mean that in five years we will be able to examine and filter bodily fluids for tiny bioparticles that reveal signs of disease like cancer before we have any symptoms, letting us know immediately if we should consult a doctor.

Information about the state of our health can be extracted from tiny bioparticles in bodily fluids such as saliva, tears, blood, urine and sweat called exosomes. These particles were first discovered in the 1980s as vesicles secreted by immature red blood cells, but later they were found to be secreted in all cell types. Scientists discovered that exosomes carry information from the cell they originated from, including proteins, RNA (ribonucleic acid) and DNA. Because the state of the originating cell can be inferred from these exosomes, they have been considered as potential biomarkers of disease for more than a decade. Besides cancer, exosomes are being considered as insightful for central nervous system diseases (Alzheimer’s, multiple sclerosis and stroke), renal fibrosis and cardiovascular disease.

At the moment it’s difficult to capture and analyse these bioparticles as they are thousands of times smaller than the diameter of a strand of human hair. But at IBM Research we are developing lab-on-a-chip nanotechnology that can separate and isolate bioparticles down to 20 nanometres in diameter, a scale that gives access to DNA, viruses and exosomes. These particles could be analysed to potentially reveal signs of disease even before we have symptoms.

This technique is known as liquid biopsy, designed to be more accessible, comfortable and convenient than the traditional tissue biopsy or cancer screening techniques many of us are familiar with. The goal is to shrink down to a single silicon chip all of the processes necessary to analyse a disease that would normally be carried out in a full-scale biochemistry lab.

In the future, the lab-on-a-chip technology could be packaged in a convenient handheld device to allow people to measure the presence of biomarkers found in small amounts of bodily fluids, streaming this information into the cloud from the convenience of their home. There it could be combined with health data from other IoT-enabled devices, like sleep monitors and smartwatches, and analysed by artificial intelligence systems for insights. When taken together, this dataset will give us an in-depth view of our health and alert us to the first signs of trouble, helping to stop disease before it progresses.

Young woman suffering from cancer

Treating a disease like cancer is expensive. According to Cancer Research UK, there was a 10% annual increase in cancer drugs prices from 1995-2013. Photograph: KatarzynaBialasiewicz/Getty Images/iStockphoto

The next five years will see significant advances in the application and development of this technology. Practically speaking, it’s difficult to say confidently that it will be in widespread use in our homes by 2022, but based on current developments and advances in research, this innovation will likely be used by physicians in some capacity by that time.

The vision for this technology is that it could be used anywhere in the world, from remote rural locations to developed urban areas. To reach the people who need it most, the technology must be affordable. Putting these capabilities on a regular silicon chip means they can be manufactured inexpensively. The challenge will be to do so at a scale that can reach people worldwide while also educating users and physicians on its benefits.

As we all know, treating a disease like cancer is expensive, and rising. According to Cancer Research UK, there was a 10% annual increase in cancer drugs prices from 1995-2013 and many new immunotherapies have price tags of over £100,000 per patient per year. When your doctor can diagnose cancer in its early stages, your odds of financial hardship – and death – decrease significantly. By making it nearly as easy to detect early stage cancer as taking a home pregnancy test, you could argue that we will change the economics of cancer. If all goes well, we may greatly diminish the physical and emotional toll of this terrible disease for future generations.

You cannot buy happiness – not even on ‘Blue Monday’ | Polly Mackenzie

Waking up today, on so-called Blue Monday, the “most depressing day of the year”, you may already be aware that this concept is based on a fraud. An almost deliciously spurious mathematical formula was dreamed up by a PR agency, given the veneer of academic rigour by attaching the name of a lecturer at a further education college, and a media phenomenon was born. More than a decade’s worth of articles and social media memes have at turns reinforced and defied the Blue Monday myth, and it is often now held up as a case study of bad science.

It is, of course, laughable to have a formula where W stands for weather and days-since-Christmas is raised to the power of Q, the days-since-we-quit-our-new-year-resolutions. But the driver behind this mockable maths is a much more sinister lie, one from which many struggle to escape 365 days of the year.

Blue Monday was originally invented for an advertising campaign for Sky Travel. Its sole purpose was to sell holidays with the false promise that spending money would raise flagging spirits. Blue Monday has now been used to sell everything from flowers to Ferraris, takeaways to airport parking. Beat Blue Monday, they tell us (laughing knowingly at the fact that it’s made up): buy something new today.

We’re told every day by advertisers that buying stuff will make us happy. A new pair of shoes will help us to feel better after a breakup. New beauty products will give us a sense that we’re “worth it”. A bigger car will give us social status. New toys will make the children happy. Even loans are sold this way: one payday lender is currently running a campaign with a smiling woman, snuggling a mug of tea and feeling happy thanks to a 1,200% APR loan, an implausible scenario if ever there was one. So it’s no wonder that on Blue Monday, the day our anxieties and misery are supposed to peak, the advertisers scream that the path out of unhappiness is paved with till receipts.

Sometimes the harm this causes is relatively trivial: overconsumption of stuff we don’t need; a bit less money in the bank; a wardrobe that won’t close because we’re not very good at throwing away our throwaway fashion buys.

But the harm can be deadly serious for people struggling with mental health problems. Imagine it: your mood is low. You find yourself crippled by anxiety. You feel like a failure, a burden on your family and friends. This is the moment when a lifetime of being told that stuff will make you feel better takes its toll: in desperation, nine in 10 people with a mental health problems find themselves spending more when they’re feeling unwell.

In my work at the research charity Money and Mental Health, I’ve lost count of the stories I’ve heard that stop me in my tracks. Mothers spending their way through months of postnatal depression – one who bought nearly 100 buggies over four years. A man who tried to buy a villa in a country he didn’t have a visa for, convinced it would turn his life around. A young woman who found buying something online was the only way she could stop a panic attack, so she did it almost every day. A husband who felt such a burden he’d buy endless presents he couldn’t afford for his wife. And many of them tell us that the boxes sit in the living room, unopened, unwanted, but impossible to ignore.

Because buying stuff is just the first part of the cycle. Next comes the guilt. People with mental health problems are far more likely to be living on a low income, and the financial damage that compulsive shopping can do is extraordinary. The boxes – or if they’re actually opened, the new possessions – are a constant reminder of the mistakes made. Too often, people don’t return unwanted goods: nearly half tell us that’s because they just want to pretend it never happened. The guilt brings their mood lower and then buying something for a temporary buzz feels like the only way back up.

We have just launched a new tool, the Shopper Stopper, which is helping people to curb their night-time shopping in particular by allowing them to set the opening hours of online shops. But it’s going to take a wider societal shift to really shake us out of these habits for good.

Comfort eating and substance abuse – other common ways to medicate mood – have physical effects. But the psychological impact of new stuff comes from our culture, not our chemistry. We can, if we change the way we talk about belongings, change the way we all think about shopping. And the people who would benefit the most are those struggling every day to find enough happiness to stay alive, and driving themselves into debt on a doomed quest to find it in a shopping basket. You cannot buy happiness – not even on “Blue Monday” – and the sooner we destroy the myth that you can, the happier we all will be.

How does your body sustain damage from a crash even when your car doesn’t?

Is it possible for a person to get injured in a car crash even when the vehicle doesn’t sustain any damage? Even if the answer is yes, auto insurance providers, for the benefit of the shareholders, do not agree with the fact. Most of them argue that as the body of the car is visibly intact, there is no way you could have been injured. This is just a generalization, but most people who’ve been involved in an accident agree that auto insurers often come up with ‘weird’ reasons just to avoid paying a claim. Such cases are tough to fight, and unfortunately, leave many victims high and dry to deal with the suffering of permanent injuries. According to car accident doctors, more than 300,000 people suffer from debilitating pain owing to auto accident injuries. Such injuries may lead to disability and prolonged pain if you do not receive the right sort of care after a car crash.

Putting Theory into Practice

TJ Szabo and JB Welcher conducted a study titled “Human Subject Kinematics and Electromyographic Activity During Low Speed Rear Impacts” to investigate the effects of a car crash on the human frame. They used live test volunteers who were subjected to a series of 10 mph crashes. The test vehicles did not suffer any structural damage. High-speed sensors and cameras were used to record the entire incident and acceleration of the cervical and lumbar spine. Owing to variations regarding gender, weight, size, height, and build, the outcomes of the research showed how the cervical spine accelerated dramatically and experienced forces ranging from 6.6g to 17.2g through the neck. On the lumbar region, the acceleration force was anywhere between 3.9g to 7.5g.  They also collected data regarding the production of acceleration before any muscle activity occurred, thereby proving that the motion was complete even before the protective muscles around the spinal cord and the spine could react and protect the body from harm.

What Does This Mean?

If a person happens to be aware of the accident beforehand, they might try to guard themselves and prepare for the impending impact, considerably decreasing the extent of the injury. However, during a real car crash, when a person gets rear-ended without any prior knowledge, the collision tends to cause a lot more damage. The muscles in the body are not strong enough to hold the head in position, thus leading to a whipping motion of the head. Such a powerful and sudden motion has the potential to cause tremendous damage to the ligaments, tendons, and muscles. Thus, in the aftermath of the crash, it is common to experience problems like neck and shoulder pain, jaw pain, and headaches. A victim sustains all these problems whereas the actual vehicle involved in the crash remains unharmed.

Extent of the Damage

Have you ever wondered why someone who’s drunk and behind the wheel often escapes mostly unhurt after a crash? The reason behind this is simple – more often than not, they’re the ones causing the accident. The entire trauma of the crash is absorbed by the car being hit. Car accident doctors who’ve been in business for years mention how they’ve seen drivers suffer unnecessarily from whiplash type injuries that have led to severe brain damage. Not many are aware of the fact that the average human head has the potential to accelerate more than 17 times the force of gravity, resulting in widespread permanent health problems.

When someone is involved in a serious accident, the force of the crash occurs within mere seconds, which is time enough to create a harsh brain injury and cause extreme damage to the essential organs of the body.

Detection of Injury

Brain injuries sustained from a vehicle collision can range from mild to severe in intensities. The less critical ones are not always evident and can escape detection by even the most trained doctors. According to the National Highway Traffic Safety Administration, the number of brain injuries occurring due to auto accidents is close to almost 7 million annually. Thus, one out of every 40 citizens is susceptible to traumatic brain injury. However, many such injuries are often hard to diagnose which is why these are considered to be silent killers.

A rear-end collision is not very uncommon. The victims of such accidents mostly sustain minor to moderate physical damage and then experiencing various symptoms such as back pain, headaches, and neck pain. The only way to find out if a car crash victim has suffered more complex but apparently unnoticeable injuries is to visit car accident doctors immediately after the accident. These doctors are trained to handle the problem of auto injuries and are capable of spotting any traumatic injuries to your brain – whether mild or severe.

When a person receives a brain injury from a car accident, it is rather easy to understand. The human brain is soft, while the skull is extremely tough which makes the brain a lot more susceptible to damage. The brain basically sloshes around within the confines of the skull, and so when the head experiences a sudden, forceful impact, some axons end up getting sheared, which ultimately is responsible for brain damage. People who’ve received traumatic brain injuries tend to experience a loss of consciousness during the time of the car crash. They might even feel groggy or nauseous when they attempt to remember the details about the accident. No matter how minor an injury to the brain is, timely and proper treatment is necessary to prevent any permanent damage and long-term symptoms.

If you have been involved in an automobile crash, it is imperative that you care for your wellbeing and get in touch with a car accident doctor who is specially trained and experienced to detect this silent epidemic. If you receive proper care on time, it might prevent the health situation from deteriorating and help save your life in the long run.

Healthcare in Afghanistan: ‘doctors are threatened at gunpoint, even by civilians’

It’s dangerous to be a doctor in Afghanistan.

This is what the staff deal with most days at a hospital in the country’s north-west: physical attacks by patients’ relatives; gun-wielding soldiers inside the wards; and verbal assaults and threats of bodily harm against doctors and nurses who are only trying to help.

An Afghan surgeon I’ve met keeps a gun at home for protection, and I understand why.

Assailants recently attacked two female nurses, causing cervical spine injuries. Another nurse responding to a mass casualty event arrived at the hospital to be assaulted and choked by relatives of a wounded patient who were demanding immediate service.

Why does this kind of violence happen in hospitals in conflict zones? There is no easy answer to that question. A patient, relative or combatant could be in pain, confused and stressed, in a life or death situation and an unfamiliar environment. They may well be angry, armed or suspicious about our neutrality. They’re living in a community consumed by conflict. But that does not mean violence, of any kind, against medical professionals is acceptable.

Doctors and nurses, ambulance drivers and paramedics, hospitals and health centres have all come under attack in Afghanistan. This disrupts the delivery of medical care when people need it most. Patients – both civilians and combatants – die because they are prevented from receiving needed care. The disruption can be so severe that the entire system collapses.

‘No weapons’ signs are routinely ignored at the hospital entrance

“No weapons” signs are routinely ignored at the hospital entrance in Maimana, capital of Faryab province. Recently I watched two soldiers, M16 rifles dangling over their shoulders, standing guard over a wounded comrade. When I asked the staff why we didn’t disarm them, I was told they were protecting their fellow soldier.

Staff don’t dare tell the soldiers to leave their arms outside and you can understand why. When a military commander was shot dead inside the hospital by opposition fighters two years ago, the doctor on duty was the one arrested, although he was later released. Arms carriers have since become even more reluctant to yield their guns before entering the hospital.

The hospital security guards once tried to disarm men entering the grounds with guns and were beaten. When a hospital administrator tried to take away weapons from a security official, the official threatened to kill the administrator. He later apologised but the deed was done.

I have witnessed these violations while working on an International Committee of the Red Cross surgical team. We are carrying out short missions to some of Afghanistan’s most important hospitals to assist the medical teams and help them learn more advanced techniques.

The Maimana hospital is fairly well equipped, and has four skilled surgeons and good hygiene standards. That’s better than many Afghan hospitals. But police officers, soldiers and even civilians there routinely threaten doctors and nurses to compel them to provide priority care at the point of a gun.

Even health workers’ families are not safe. A surgeon’s child was briefly kidnapped just a month ago. Another doctor’s child was kidnapped a few months earlier, forcing the doctor to sell his home to pay the ransom. One surgeon and his family were so afraid of the recurrent threats that they simply packed their bags and left.

This all leads down a dangerous, deadly path.

My message to community leaders, fighters and even average citizens is clear. It is the same message that my colleagues are using in conflict zones across the world: if you don’t respect and protect the healthcare workers, you will soon find no one left to care for you.

Wars must have limits, so respect the laws of war. Respect hospitals and the staff working in them. Let them do their jobs.

Healthcare in Afghanistan: ‘doctors are threatened at gunpoint, even by civilians’

Don’t blame the fashion world for the cult of skinny – even Roald Dahl plays his part | Hadley Freeman

It’s September, which means – exciting! – it’s fashion Groundhog Day again. This consists of fashion weeks starting up, followed swiftly by the announcement of a political initiative to protect women against this evil on the catwalk. (Another feature of fashion Groundhog Day, incidentally, is a slew of articles by male columnists expressing bafflement at why the models are so thin, which can be summed up as: “I don’t find this sexy, so why?!” However would we know how to look without men telling us what turns them on?)

Anyway, the latest initiative comes courtesy of the Women’s Equality party, which has launched a campaign demanding, among other things, that British designers and retailers stop using size zero clothes in shows and photoshoots. This, the WEP claims, “will tackle negative body image issues and eating disorders”.

I’m going to ignore the all-too-predictable connection between fashion models and eating disorders here, because I’ve written enough for this lifetime about how absurdly reductive it is to suggest that a mental illness is caused by Vogue. Instead, I’ll say this: the WEP is perfectly within its rights to address this issue, but – spoiler alert – it will not make a blind bit of difference. There have been efforts to legislate against fashion’s obsession with skinniness in the past; these laws, wherever they’re passed, always get a lot of play, because the media love a story that allows them to run a photo of a skinny model at least as much as the fashion industry likes the skinny model herself.

So it would be understandable if you were confused as to why things haven’t changed. The problem is not only that the laws are rarely enforced, but also that they are the equivalent of rearranging the Titanic’s deck chairs. For a start, fashion is too international for one country’s legislation to make a difference – especially, I’m sorry to say, when it’s this country doing the legislating. With the exception of Burberry, British-based fashion companies simply do not spend enough on international advertising for any laws to have an effect; the rest of the world won’t even see the photographs. Slapping legislation on a tiny British brand will not change the aesthetic if Chanel and Prada can carry on as before.

While fashion represents the connection between skinniness and perfection in its purest form, it is also the end point. By the time a woman is looking at fashion photography, she’ll have gone through years of indoctrination that the less of her there is, the better. It’s there in all the children’s books and films, in which the evil women are plump and the nice ones are slender (Roald Dahl is especially bad on this). It’s there in pretty much every film ever made (the only person I’ve interviewed with an evident eating disorder was not a model but an actress). And it’s there in the general atmosphere of being female today, the things you grow up hearing the adult women around you saying about their bodies, the way they decline dessert. According to a report just published by the Children’s Society, teenage girls are unhappier than ever, especially with their bodies, which they compare unfavourably with those of their friends and celebrities.

This masochistic tendency towards self‑erasure is a complicated issue, something a well‑meaning but overly simplistic campaign against thinness in fashion can’t fix. The WEP has also suggested including body-image lessons as part of the school curriculum, which is a better idea (and, tellingly, it has had less pick-up, what with it lacking the vital excuse for a photograph of a model). But instead of fussing about the meaningless phrase “size zero”, or conflating thinness with anorexia, the WEP would be more effective if it looked at the prevalence of eating disorders in the fashion industry and exposed this. Instead of pretending it can tell fashion editors what to put in their magazines, the WEP could talk to them to get a more realistic sense of the problem, and get them on side.

If we really want to end the association between female skinniness and female aspiration, women need to be doing this on an individual, focused level, not leaving it to politicians to act on an amorphous, collective one. Look at the way you talk about your body and what you eat, especially in front of young girls. Call out publications that condescend to larger women and feature photographs only of slim ones. Remind your female friends and your daughters that their jeans size is not a measurement of their personal value.

It is easy to damn the fashion industry for promoting skinny as the feminine ideal. It’s harder to admit that it is only echoing back too many of our own darkest thoughts, our own self-loathing.

Function / Existence Balance -- Is It Even Attainable?

I not too long ago go through an intriguing review published in Harvard Company Review about how productive executives balance operate and daily life. Primarily based on 5 years’ worth of interviews with virtually 4,000 executives throughout the world, the research demonstrates 5 major themes amongst these executives who most properly mix their operate and home lives:

1)   Defining Accomplishment for Yourself – The initial step to making complete existence alignment is to define what achievement means to you, and stay correct to your values amidst tempting delivers. Your values might shift above time, but it is critical to be clear on what success means to you nowadays and honor that reality. Sometimes this could indicate your progress is a minor slower on a specific undertaking at perform since you want to spend time with family, other times this might indicate you miss out on time with your spouse because you acquired an invitation to an unique perform occasion. 1 useful device to get clear on your values and define what good results indicates to you is this Values Physical exercise.

Resolution:

  • Get clear on your values and then make deliberate alternatives that align with your values. This is how you prioritize routines and start off to make sense of the conflicts that will inevitably arise.

2)   Managing Engineering – Leaders who effectively reside a balanced daily life are much better able to set boundaries with engineering and stay away from distractions. The capacity to target on a single process for a certain of time – whether or not that activity is investing good quality time with your young children or cranking out a presentation for your Board – is one particular of the keys to generating a wholesome perform/lifestyle stability. Undivided focus is critical. Otherwise, you are not only significantly less productive, but you will also truly feel far more psychologically drained as a consequence of what Brigid Schulte refers to as “time confetti” – that feeling of getting in many locations at when in your personal thoughts.

Answer:

  • End multi-tasking and do Energy Hours instead.

  • Set clear boundaries with your crew about your availability (ex: I will not be on the web from 6:30 – 8pm each evening when I’m with my family).

  • Bulk tasks by manually downloading your e-mail and checking it only 3 occasions a day.

  • Block out at least one particular hour a week for inventive, substantial level thinking. One executive I know has a meeting with “Dave Johnson” every week on the calendar….no one is aware of that meeting Dave Johnson entails going on a walk or sitting in a coffee store carrying out massive image pondering.

three)   Constructing Support Networks at Work and at Property – In order to come to feel relaxed and balanced, it is critical to have plenty of competent support the two at work and at residence. Emotional help in certain is essential the two at operate and at property. Becoming able to confide in other folks allows a individual to vent, really feel more genuine, and receive advice from a trusted supply. And, according to the HBR Examine, “Many leaders reported that health crisis – their personal or a household members’ – might have derailed their careers if not for compassionate bosses and coworkers.” There will be tough moments, sudden crises, and it requires community and support to move by way of these demanding times and into calmer waters.

Solution:

  • Take Inventory – ask oneself, “hmmmm, where am I most lacking help proper now?” Make a commitment to retain the services of or delegate more in that specific region. Maybe it’s receiving a person to clean your residence each week instead of each other week so that you by no means have to worry about laundry, or maybe it’s paying an hour education 1 of your staff so they can get more than a weekly activity. One interviewee stated, “We employ people to do the a lot more tactical things—groceries, cooking, assisting the kids dress—so that we can be there for the most important things.”

four)   Traveling or Relocating Selectively – Numerous leaders surveyed think in traveling much more extensively when they’re young and significantly less encumbered. Of those surveyed, 32% said they had turned down an worldwide assignment because they did not want to relocate their families, and 28% stated they had completed so to shield their marriages.

Resolution:

  • It is okay to Just Say No. Occasionally, you may pick to pass on an opportunity simply because of the strain it would put on your existence outside of function. Stand your ground and return to your values. If in doubt, remember that more than a third of these effective executives had to make the very same decision and turned it down. You are not alone, and you will not drop your livelihood by turning down 1 possibility. If you produce a three Year Vision and emphasis on what your achievement actually is, new possibilities will show up.

5)   Collaborating with Your Spouse – It’s important that you and your companion have a shared vision for accomplishment. A accurate collaboration is a single in which every personal is getting his or her needs met. Many of the leaders surveyed “consider emotional help the most significant contribution their partners have made to their careers. They also seem to their partners to be sounding boards and truthful critics.”

The research also pointed out “Interestingly, Males, nevertheless, appear to be acquiring more spousal support general. Male interviewees – a lot of of whom have keep-at-house wives – frequently spoke of their spouses’ willingness to get care of children, tolerate long operate hrs, and even relocate, occasionally as a way of life.

“Women, by contrast pointed out their partners’ willingness to totally free them from conventional roles at property. 1 said ‘He understands the demands of my function and does not place stress on me when perform takes far more time than I would like.’ Male executives have a tendency to acquire much more assistance at property, whilst female executives praise their partners for not interfering with their function.”

Resolution:

  • Marry someone who shares your career vision. Have these conversations before you stroll down the aisle.

  • Truly, the answer to this one particular is much more complicated….I would propose hunting to Denmark as an illustration of how to assistance working families.

In summary, whole daily life alignment is possible, and it does demand creating conscious selections. It is crucial that you define what good results signifies to you, set boundaries with technological innovation, build robust support networks, discover to say “no” to comprehensive travel or relocation, and uncover a partner who supports you emotionally.

A proof of idea for an HIV cure exists. Now even our setbacks are beneficial | Françoise Barré-Sinoussi and Sharon Lewin

It was a sad and disappointing turn of events: the kid known as the Mississippi child, in remission of HIV infection for much more than two many years right after discontinuation of antiretroviral treatment (Art), now has clear evidence of HIV rebound. The announcement will undoubtedly be viewed by some as a setback in the renewed quest for an HIV cure.

It doesn’t want to be. We require to take the highs and lows in our stride. We have a proof of idea that a cure for HIV is attainable in the Berlin Patient, Timothy Brown, who acquired a bone marrow transplant and remains virus free off therapy for more than six years.

We also have the situation of the French Visconti cohort, now comprising twenty HIV-positive men and women who have been taken care of very early with HIV medication, but then stopped treatment. Right after virtually ten many years off therapy, they are keeping the virus under tight manage.

Only one person, who is now in remission for a non Aids-connected cancer, reinitiated Artwork despite his HIV infection even now currently being controlled at the time that he obtained anti-cancer therapy.

The lows – if the Mississippi infant case can be called a reduced – provide main insights into the scientific problems that lie ahead for the researchers, scientists and others gathering at the Aids 2014 symposium in Melbourne this week.

Comparable to the Mississippi infant, the Boston patients – two guys who received bone marrow transplants that appeared to rid them fully of HIV – also relapsed, and are now back on antiretroviral treatment.

What has been most encouraging for all three sufferers was that the time the virus stayed beneath management off remedy, or in remission, was drastically longer than what we have ever witnessed before. The Boston patients were virus-free of charge for 12 and 32 weeks, and the Mississippi infant virus-totally free for a impressive 27 months.

Nevertheless, as we have discovered above the past 3 decades, HIV has many tricks up its sleeve.

An minute sum of virus – which none of our present exams are even shut to detecting – was capable to persist in these 3 patients and reappear at any time, with absolutely no warning.

The Mississippi baby, and the Boston sufferers, tell us that to attain extended phrase HIV remission we will likely need to have to tackle the difficulty on multiple fronts – lowering as much as attainable the quantity of extended-lived, latently contaminated cells existing in the entire body, as effectively as bolstering the host defence. 1 can not be carried out without the other.

We have usually known that the search for an HIV remedy wasn’t going to be easy. We have to be optimistic, and hope that situations like the Mississippi infant will demonstrate to be one more piece in the jigsaw puzzle that is an HIV remedy.

To attain this objective will need substantial academic collaboration as nicely as public-private partnerships that can drive innovative and effectively-funded strategic research over the prolonged term.

We have to proceed the search for an HIV remedy. We owe it to the 35m men and women living with HIV.

Even With Cancelled Policies, Uninsured Numbers Falling Quickly

Evidence is mounting that the Cost-effective Care Act is operating to significantly reduce the quantity of uninsured Americans, despite inquiries about regardless of whether Americans had been buying policies just to exchange individuals that had been cancelled.

On leading of yet another U.S. government report Friday exhibiting far more Americans receiving coverage, especially from the Medicaid health insurance program for the bad, the latest from the Urban Institute’s Overall health Reform Monitoring Survey was clear: the “number of uninsured adults continues to fall underneath the ACA.”

However numbers vary, the Urban Institute says the variety of uninsured is down 8 million as of June.  “Though estimates of the dimension of the net acquire in coverage vary across surveys, there is constant evidence of ongoing gains in insurance coverage coverage beneath the ACA,” authors of a new Urban Institute report, linked right here, demonstrate.

The Urban Institute report, along with other folks that have been launched in recent weeks seem to undermine the argument from opponents of the law who wonder no matter whether Americans have been merely changing cancelled policies with those they were offered under the hwalth law. Numerous million Americans had their policies cancelled due to a portion of the law that calls for personal policies to have specific vital overall health rewards. (See associated gallery, also linked here)

The Urban Institute mentioned its survey didn’t account for potential other impacts that might reduced the uninsured rate such as how many young adults may well be taking benefit of a single of the law’s earliest positions enabling dependents to keep on their mothers and fathers health strategy till they are 26 years old.

But the law is doing work for youthful grownups and kids, according to a report launched Friday from Cindy Mann, deputy administrator of the Centers for Medicare &amp Medicaid Companies and Director of the Centers for Medicaid and CHIP Services.

Her report explained six.7 million more folks – an eleven.4 percent increase –  were enrolled in Medicaid applications for the poor or the Children’s Well being Insurance System as of Might 31 in contrast to “the baseline period from July though September 2013.”

The overall health law offers a money infusion of more than $ 900 billion in federal bucks from 2014 to 2022 to increase Medicaid applications for states that agreed to take advantage of the proposition.

This Medicaid funding under the Cost-effective Care Act is in contrast to past efforts to expand Medicaid in that the federal government will choose up the complete tab for the very first three many years. The state gradually has to pick up some charges in 2017 but by 2020 the federal government is even now selecting up 90 % or far more of the Medicaid tab.

It is a much greater funding proposition than the present Medicaid program that basically shares the expense among the states and the federal government. The federal government historically picks up a small much more than half of the value of Medicaid.

States usually led by Republicans that opposed this expansion haven’t seen almost the growth in Medicaid enrollment as 26 states which includes the District of Columbia that adopted and implemented the Medicaid expansion. “Enrollment in people states rose by 17 percent even though states that have not expanded reported only a 3 % enhance,” Mann wrote in her report.

When it comes to Medicaid, enrollment is expected to proceed to grow because it doesn’t have an open enrollment time period like the private coverage supplied on government run marketplaces acknowledged as exchanges. The next open enrollment period for private coverage is later this fall when a number of insurers, this kind of as Aetna (AET), UnitedHealth Group (UNH) and an array of Blue Cross and Blue Shield ideas are providing coverage in even much more markets than they did this previous open enrollment period.

In the meantime, Americans can still apply for Medicaid coverage.

“Consumers may still apply for Medicaid and CHIP coverage 12 months round and, as soon as enrolled, can use this coverage right away,” Mann explained.

Why J Crew is promoting clothes that"s even smaller than dimension zero

Waisting away

Waisting away? No healthful grownup ought to need to have a size 000. Photograph: Alamy

Title: 000.

Age: Brand new.

AKA: XXXS.

Physical appearance: Really modest.

How modest? If it were a particular person, it would have a 58cm (23in) waist.

Is it a particular person? Allegedly.

If it is not a man or woman, then what is it? It really is clothing. Or, far more exclusively, it’s a clothing dimension. Or, even far more particularly, it really is a women’s clothes dimension just lately introduced by US retailer J Crew.

Who the hell is supposed to match it? Officially, J Crew is making 000 clothes to appeal to the “smaller sized-boned” female. But it has been accused of taking “size vanity” to preposterous extremes.

Dimension vanity? It means producing the illusion of thinness in your customer by resorting to quantity deflation. The notorious American “dimension zero”, for example, equates to a United kingdom dimension 4. A minor later, the 00 dimension (United kingdom 2) was introduced, now followed by 000.

Doesn’t that simply help to perpetuate a warped public perception of what constitutes a regular human physique? It undoubtedly does. A 58cm waist is deemed a healthy measurement for a woman aged six to eight.

It really is the two harmful and sickening! Your outrage is mentioned, even though dimension vanity is typically for the advantage of the bigger buyer, ie the size 24s who have been flatteringly downgraded to a twenty.

Is that J Crew’s explanation? Oh no. A spokesman for J Crew stated the 000 innovation was “basically addressing the demand coming from Asia for smaller sizes than what we had carried. Our sizes typically run large and the Asia market place tends to run little.”

Now I do not know what to believe. Or what to wear. Mail Online, alternatively, attributed the demand for the new sizing to a trend on the component of female celebrities of posting skeletal selfies, which are noticed by impressionable young ladies.

I’ve in no way seen any such selfies. Will not fret, Mail On-line reprinted loads of them so you can get the notion.

What next? Zero-zero-hrs contracts? Treble-zero tolerance? Never give anybody any suggestions.

Do say: “It does not even make mathematical sense – zero occasions zero occasions zero is nevertheless zero. Zero to three decimals areas is still zero. YOU Cannot HAVE Smaller sized THAN ZERO.”

Will not say: “Hey, skinny! The children’s division is on the second floor.”

Staying Productive Even though Staying Lively

In accordance to the Centers for Disease Control and Prevention, the percentage of Americans who are obese is…are you prepared for this… a staggering 69%. Allow me repeat that, 69%. As in over two-thirds. As in practically 7 out of 10 men and women.

Note to Self: Keep in mind to work out. I’m one of the 69%.

New Note to Self: Make time to operate out. Of program I would operate out much more frequently if I had the time.

Query to other workaholics: How do you make time to workout in a schedule that you can’t often handle?

There are several answers to this dilemma that almost all of us, in one particular guise or yet another, bargains with. But one remedy comes in the type of the modifications that are occurring in the workplace. I’m detecting an increasing buzz toward reinventing the workplace—in element to promote overall health and wellness. Folks want to break out of traditional office designs that restrict mobility and are centered on the concept of a desk and a chair. That is so last century—and it feels precisely like the walled gardens that I worked in earlier in my occupation.

I had a tour of Motorola’s new headquarters facility at Chicago’s iconic Merchandise Mart, and I couldn’t feel what I saw. For a single, they don’t have landline phones. Every person was strolling close to with Bluetooth headsets on and our tour manual said that staff fairly a lot use Google Google Voice for their perform quantity. I also saw a ton of flexible operate room, cushy pillows, plenty of food—junk and otherwise—and one of the nicest decks you’ll ever see (on top of the Merchandise Mart). Of course they wouldn’t let me consider any photographs (on my Samsung Galaxy S5), and as you can see from this authorized photo spread, the workspaces are not at all conventional and one guy has a treadmill desk so he can walk although operating.

This Forbes write-up from last 12 months demonstrates ten workout routines you can do at your workplace with just a normal desk and chair—no devices, application or contraptions necessary.

Scientific studies have proven that an alert body and active way of life boosts not just productivity, but also happiness and satisfaction levels. Normal office employees commit more than six hours a day sitting at their desks and staring at their personal computer screens. There is now an explosion of research scientific studies that show the damaging results of sitting all day.

Not surprisingly then, a quantity of startups are focusing on developing goods and devices that get at the sedentary nature of traditional function. That is, sitting.

If you do not have time to operate out, then why not adapt your workspace so you can workout even though working? At the largest office furniture trade present in the U.S., Neocon, held final month also at the Merchandise Mart, numerous exhibitors had been exhibiting off innovative new answers geared toward health and wellness in the workplace. At Neocon one could see treadmill desks, standing desks like the Stir Kinetic desk (similar to desks which I very first saw at Argonne National Laboratory a handful of many years in the past), bike desks and progressive and a lot reduced price answers like the Cubii from Chicago-based commence-up FitnessCubed. Cubii is an beneath-desk elliptical trainer, that does not call for any adjust to existing furnishings, which makes it possible for office dwellers to keep lively even though they sit at their desks. Cubii comes with its very own mobile app, but also syncs with current fitness trackers. “A variety of firms were creating fitness trackers, but we desired to make anything distinct, so we produced a source of workout to complement fitness trackers,” says Arnav Dalmia, FitnessCubed’s CEO.

Cubii

What I’ve personally identified about bodyweight reduction is that it is much less complicated to locate your determination and stay the course when your progress is becoming reinforced. A number of years ago I received a Withings Wireless Scale. Each and every time I step on the scale, my fat is captured electronically, and I can view my progress from an app on my smartphone. It was a massive assist to me it achieving my fat reduction goals—much of which I put back on after soothing my discipline.

Whilst retaining active in the workplace is a huge component of workplace wellness, it is not the only one particular.  Retrofit, a corporate weight reduction firm also primarily based in Chicago, has made an app to complement your fitness tracker. The business provides extra encouragement by means of reside, on the web education sessions with wellness teams consisting of registered dietitians, behavior coaches and workout physiologists. While not specifically in the class of products, it nevertheless is supplying a useful workplace solution for time-starved workaholics, et al.

The obese issue in the U.S. is at crisis amounts, no doubt exacerbated by poor diet plans, but a contributing element is the rather sedentary nature of knowledge-based work. It’s no shock, then, that innovation is coming to the workplace to deal with this perpetual problem.

Even With Sovaldi And ACA Expenses, Health Inflation To Rise "Modest" 6.eight% In 2015

The launch of new Hepatitis C therapies like Sovaldi and a flood of new wellness care buyers, thanks to an enhanced economic system and the Affordable Care Act, will aid push overall health investing up six.eight % in 2015, in accordance to a new study.

But researchers at PwC’s Well being Study Institute say employer approaches to shift costs to staff by way of buyer-directed health plans, savvier well being care buyers usually and the increasing shift away from fee-for-services medicine must preserve health inflation from jumping by double-digit percentages frequent in the late 1990s via significantly of the early 2000s.

“There are a whole lot of factors taking place that are trying to keep medical expense trend in check out,” said Rick Judy, Principal in PwC’s Wellness Industries Advisory practice in an interview with Forbes.

Judy referred to as overall health care suppliers gaining efficiencies “systemness” whereby they are streamlining administrative functions even though strengthening clinical care by moving away from charge-for-services medicine in which doctors and hospitals are paid for each and every remedy to other models like accountable care organizations (ACOs).

In contrast to traditional fee-for-services approaches that can lead to extreme or unneeded treatment method, accountable care arrangements like ACOs group suppliers under an umbrella entity that generates finanicial rewards from insurers when physicians and hospitals function with each other to boost top quality.

“Large hospital methods are commencing to operate in a far more efficient and successful way,” Judy mentioned. “The client is much more value-conscious.”

Different varieties of health care providers are far more “value-based,” he extra. The jury stays out on ACOs but they are commencing to “bare fruit” in the kind of price savings that are employed to spend additional funds to companies.

But there are some unknowns ahead for the U.S. wellness care technique that are adding to wellness costs, particularly in the region of prescription medicines.

PwC’s Wellness Study Institute says new specialty prescription medicines like the Hepatitis C drug Sovaldi offered by Gilead Sciences (GILD) will contribute .two percent to complete paying development next year.

Sovaldi and other Hepatitis C drug coming on the market in the next 12 months from Abbvie (ABBV), the drug division spun off final yr from Abbott Laboratories (ABT) and Merck (MRK) are hailed for their effectiveness in treating Hepatitis C, a condition that effects a lot more than three million people in the U.S.

But Sovaldi is also high-priced at $ one,000 a pill, or a lot more than $ 80,000 for a program of therapy per patient.

“It’s just a very fascinating dilemma for the sector,” Judy stated.
