Premature baby saved by groundbreaking NHS surgical team

A “miracle” baby girl has spent her first two nights at home after becoming what her doctors believe is the world’s youngest patient to survive major abdominal surgery.


Abiageal Peters is expected to lead a normal healthy life after being saved in pioneering operation her parents and doctors say was only possible under the NHS.


“She is always going to be our little miracle,” her mother, Louise Peters, said from her home near Esher, Surrey, with the sound of Abiageal gurgling in the background.


Abiageal was born in October 2016 at St Peter’s hospital, Chertsey. She was three months premature, after a gestation of only 23 weeks. Peters was warned her baby had little chance of overcoming a severe gut condition known as perforated necrotising enterocolitis. Her intestine was torn in three places and her stomach had started turning black.


An operation at a specialist neonatal unit at St George’s hospital, London, was Abiageal’s only hope, despite the risk of surgery on such a young and small baby. She weighed just 609g (1lb 5oz).


“She would have been dead without the surgery,” Peters said. “She was deteriorating so badly at that point. She was struggling to breathe and her skin was turning black. We knew she had a day or two left if that, if they didn’t operate.”


Consultant Zahid Mukhtar, who led the operation, said it was extraordinary that Abiageal managed to pull through surgery. “We have trawled the literature and couldn’t find anyone who has been operated on at that early an age and survived. Her story and her recovery has been really remarkable.”


Speaking to the Guardian, he said: “She was smaller than my hand when we operated on her. But it was her prematurity that was so unusual – her organs were so fragile and jelly-like that as soon as you started operating, even with our small instruments, everything started to bleed or fall apart.”


He spoke of the team’s pride in saving Abigail. “It is beyond the craft of surgery, something more has to come into it, to push boundaries like this. It is an amazing achievement for our team, so we are really proud. The fact that four months later she is a really healthy child, with a good brain, is amazing considering how premature she was.”


Peters, a financial analyst who grew up in South Africa, said: “We can’t thank the doctors enough. I don’t know if she could have survived outside the NHS. Some countries don’t even think about helping babies who are less than 25 or 26 weeks. She would have had very little chance.”


“I come from a country where you have to pay for every cent of your medical aid. This country has been phenomenal. I think the NHS is absolutely wonderful.”


Peters and her British husband, David, also have a two-year old daughter, Tara. “My first daughter was such an easy pregnancy that I just took it for granted that my next baby would be as easy,” she said.


Peters added: “The beginning was especially stressful, but my baby is a fighter. Then it was four months of waiting for her come off breathing support and sort out her feeding. There have been lot of hurdles, infections, blood transfusions along the way.


“Other mothers who have this happen to them, should know there is a chance – don’t give up, these little babies are so strong.”


She added: “It has been wonderful to have her home, but there are all the usual niggles of getting a new born to sleep.”


Mukhtar said the success of the operation demonstrated the NHS at its best.


“Despite the financial pressures and morale challenges, the staff working in the NHS are amongst the best in the world,” he said. “They are really pushing the boundaries. Any patient that comes into our system gets the best we can do for them. It is mindset that is different from other places in the world where I have worked. It is a real honour for me to me working in this system.


“Outside the NHS there is no chance she would have survived. Any child in a private hospital born this prematurely would have been transferred to the NHS immediately.”


Mukhtar, who was born in mountain village in Pakistan, said he wanted to become a doctor after his three-year-old sister died from diarrhoea. “From a simple thing like that she died, because there was so little help for sick kids. From then I have tried to never give up on a child and do whatever is possible.”


He added: “I was a village kid with uneducated parents and yet this country gave me the opportunity to follow my dream and now I am able to give back that education and possibility to affect people’s lives so profoundly. It is a real shame that people are so suspicious of people coming here from other countries.”

Millions of premature births could be linked to air pollution, study finds

Air pollution could be a contributing factor in millions of premature births around the world each year, a new report has found.


Nearly 15 million babies are born annually before reaching 37 weeks gestation. Premature birth is the leading cause of death among children younger than five years old, and can cause lifelong learning disabilities, visual and hearing problems, the World Health Organization (WHO) reports.


Researchers for the Stockholm Environment Institute (SEI), the London School of Hygiene and Tropical Medicine and the University of Colorado, have concluded that as many as 3.4 million premature births across 183 countries could be associated with fine particulate matter, a common air pollutant, with sub-Saharan Africa, north Africa and south and east Asia most impacted by the issue.


“Preterm birth and associated conditions are one of the biggest killers of children in the US and worldwide,” said Dr Paul Jarris, chief medical officer at the March of Dimes, a US-based nonprofit focused on maternal and baby health. “Yet, there’s a lot of things we don’t know about what causes preterm birth, so every bit of information we can get is helpful.”


“We have known for a long time that air pollution contributes to asthma and heart disease in adults,” said Jarris. “What I think people fail to recognise is that so many of these risk factors impact babies before they are even born.”


Previous studies have looked at how effects of air pollution in utero might negatively impact babies’ birth weight, or the likelihood that they will be born early. SEI’s study, which examined data from 2010, attempted to calculate how those factors might influence the global rate of premature births.


“By showing in our study that 18% of preterm births are associated with air pollution, we are quantifying the health impacts of fine particulate matter on babies before they are born,” said Chris Malley, a researcher in SEI’s York Centre, at the University of York and lead author on the study.


Each year, around one in every 10 babies worldwide are born prematurely, according to the WHO. Africa and south Asia bear a disproportionate burden of premature births, accounting for 60% of all premature births globally. That region also dominated SEI’s report of premature birth associated with air pollution.

SEI’s new report provides an estimate of potential birth impacts associated with air pollution, but the authors acknowledged that the study had considerable caveats because of a lack of research in some of the most affected areas.

Still, two experts who reviewed the study called estimates “conservative”.


A number of factors have been identified in playing a part on premature birth, including poverty, infection, smoking and substance use, physical activity and maternal education. However, even with the report’s limitations, it is still one of the first to argue that reducing air pollution could also be effective in reducing premature births across the world.


Because of a lack of research in regions such as south and east Asia and Africa, researchers used studies conducted in the US and Europe to estimate global outdoor air pollution exposure.


Indoor cooking fires could play a larger role than the study allows for. “The exposure in Asia, and in part in say, China and India, to outdoor air pollution is significantly higher than outdoor air pollution in the US,” said Malley. “So that’s the other part of the uncertainty, that at these higher levels of exposure we don’t know whether this same relationship holds.”


The report focused on one kind of air pollution considered especially harmful to human health: fine particulate matter. This pollution is made up of tiny particles from a variety of emissions, such as diesel emissions and agricultural fires.


The particles, smaller than 2.5 micrometres in diameter, are considered harmful because they can lodge deep in the lungs, affecting the pulmonary and cardiovascular systems. Past studies have looked at these factors as ways that air pollution may impact preterm birth and low birth weight in babies.


South Asia had the most preterm births associated with particulate matter pollution – as many as 1.6 million associated births. Notably, even though China has a relatively low rate of preterm births, researchers found that as many as 521,000 preterm births could be associated with air pollution because of high concentrations of particulate matter.


A similar study conducted in the US found that air pollution had a costly impact on unborn children, estimating that the economic impacts cost $ 4.33bn in 2010.


Researchers said the estimates support their conclusion that “reduction of maternal [air pollution] exposure through emission reduction strategies should be considered alongside mitigation of other risk factors associated with preterm births.”


“This is one more reason why we need to be good stewards of the environment,” said Jarris. “The most vulnerable among us – unborn children – are affected, and really in a way that impacts families’ lives for generations.”

Very premature babies at risk of mental health problems – research

Children who are born very prematurely are at greater risk of developing mental health and social problems that can persist well into adulthood, according to one of the largest reviews of evidence.


Those with an extremely low birth weight, at less than a kilogram, are more likely to have attention disorders and social difficulties as children, and feel more shyness, anxiety and depression as adults, than those born a healthy weight.


The review draws on findings from 41 published studies over the past 26 years and highlights the need for doctors to follow closely how children born very prematurely fare as they become teenagers and adults.


“It is important that families and doctors be aware of the potential for these early-emerging mental health problems in children born at extremely low birth weight, since at least some of them endure into adulthood,” said Karen Mathewson, a psychologist at McMaster University in Ontario.


Improvements in neonatal care in the past two decades mean that more children who are born very prematurely now survive. In a healthy pregnancy, a baby can reach 1kg (a little more than 2lbs) within 27 weeks, or the end of the second trimester.


The study, which involves data from 13,000 children in 12 different countries, follows previous research that found a greater tendency for very low birth weight children to have lower IQs and autism and more trouble with relationships and careers as they reach adulthood and venture into the world.


It is unclear how a very premature birth, known as a preterm, affects brain development, but children who survive the difficult start are consistently found to be more introverted and risk averse. These factors may drive the more positive tendency to become less prone to drink, smoke and take drugs as adults. The increased risk of mental health problems was seen in very preterm children regardless of where they were born.


Children who were delivered extremely early and weighed less than a kilogram at birth were about four times as likely as those born at term to have attention deficit hyperactivity disorder (ADHD), and significant emotional problems. Those who reached adolescence were twice at risk of these. Adult survivors reported more mental health and social problems, but Mathewson said there are far fewer studies on these individuals.


“This does not mean that, in general, infants born extremely preterm will ultimately develop mental health problems, only that the risk of developing such problems is higher in this group than in those born at full term,” she stresses in the journal, Psychological Bulletin.


Daniel Smith, professor of psychiatry at the University of Glasgow, said the findings were important because mental health issues that occur in childhood are a strong predictor of psychiatric disorders in adulthood.


“There is a strong case for assessing, on a regular basis, the mental health status of these children, so that early intervention approaches might be implemented sooner rather than later, with a view to minimising future mental health problems,” he said. “It is my understanding that children who are born with an extremely low birth weight are routinely assessed for physical health problems in childhood but not currently for mental health problems. This paper suggests that this situation should change.”


Dieter Wolke, a psychology professor at the University of Warwick, said children who weigh less than a kilogram at birth have a “distinct profile” of mental health difficulties in childhood and adolescence. But it was important to note that a third to a half of the children will grow up without any of these mental health problems, he added.


“This study further underlines that extremely low birth weight children and their families need more support to deal with, or to reduce, the adverse effects of ADHD, anxiety and social problems that affect their schooling, being part of their peer group, and being socially integrated. Our own findings indicate that these mental health problems affect wellbeing, wealth and finding a partner and friends who are supportive into adulthood,” Wolke said.


A new EU research programme, coordinated by the University of Warwick, will explore what helps very preterm children overcome the problems they face. Better support for parents and schools, where teachers can learn about the children’s special needs and how to handle their difficulties with attention and making friends, can all help, studies show. “What has been repeatedly noted is that after they leave hospital, they require better liaison of services in the community and with educational services to improve their lives,” Wolke said.

"Kangaroo care" makes premature babies healthier and wealthier, study finds

Premature babies who were breastfed exclusively and kept warm through continuous skin-to-skin contact have become young adults with larger brains, higher salaries and less stressful lives than babies who received conventional incubator care, according to a study published this week.


The research (pdf), in the journal Pediatrics, compared 18- to 20-year-olds who, as premature and low birth-weight infants, were randomised at birth in Colombia to receive either traditional incubator care or kangaroo mother care (KMC) – a technique whereby parents or caregivers become a baby’s incubator and its main source of food and stimulation – until they could maintain their own body temperature.


The kangaroo method involves the baby nestling in a “kangaroo” position on the caregiver’s chest as soon as possible after birth, accompanied by exclusive breastfeeding. Parent and child leave the hospital together as soon as possible after birth, after which there is rigorous monitoring of baby and mother for one year after the infant’s original due date (rather than the actual birth date).


Researchers investigated 264 of the KMC participants who weighed less than 1.8kg at birth, and found that the technique offered significant protection against early death. The mortality rate among incubator-treated babies was 7.7%, more than double that of those in the KMC group (3.5%). Almost every other area investigated revealed further advantages: average hourly wages of the KMC group were nearly 53% higher than their counterparts; cerebral development was significantly higher; family life was found to be more nurturing and protective; and children spent more time in school and were less aggressive, hyperactive and stressed.


“This study indicates that kangaroo mother care has significant, long-lasting social and behavioural protective effects 20 years after the intervention,” said lead researcher Dr Nathalie Charpak, of the Kangaroo Foundation in Bogotá.


“We firmly believe that this is a powerful, efficient, scientifically based healthcare intervention that can be used in all settings, from those with very restricted to unrestricted access to healthcare.”


According to the World Health Organisation, nearly one in 10 babies worldwide is born preterm (before 37 completed weeks of gestation), with resulting birth complications the leading cause of death among children under five. Preterm birth rates are rising globally every year, yet more premature babies are born in low-income countries (9%) – where they face a greater risk of complications – than high-income countries (12%). In Malawi, for example, 18 in every 100 births are preterm.


Many survivors face a lifetime of disability – including learning disabilities and visual and hearing problems – and require extra care to avoid illness and death from secondary, preventable complications including hypothermia. In developing countries, where incubators are often scarce and unreliable, kangaroo mother care could save lives, said Dr Peter Singer, chief executive officer of Grand Challenges Canada, which supported the research.


“A premature infant is born somewhere in the world every two seconds,” he said. “This study shows that kangaroo mother care gives premature and low birth-weight babies a better chance of thriving. Kangaroo mother care saves brains and makes premature and low birth-weight babies healthier and wealthier.”


While the technique does not replace modern science or neonatology, it is an excellent complement, said Charpak. Hospitals in Scandinavia, among them the NICU in Uppsala, Sweden, are using KMC to stabilise preterm babies. Grand Challenges Canada is funding two “centres of excellence” and 10 treatment centres to deliver kangaroo care across Cameroon and Mali, where preterm birth rates are among the highest in the world.


The study’s positive findings are impossible to attribute to one reason alone, said Charpak. Rather, they result from a multidisciplinary approach involving regular skin-to-skin contact, breastfeeding, education of the mother and family, and support over a 12-month follow-up period.


“One of our hypotheses is that, by placing the infant in the mother, father or caregiver’s chest, the infant’s brain is in a less stressful environment,” said Charpak. “KMC also creates a climate in which the parents become progressively more aware of the child and more prone to sensitive caring.”


In contrast, said Charpak, a preterm baby born at 30 weeks could spend seven weeks in an incubator, where it is separated from its mother and faces a steady stream of light and noise. “It is easy to understand why this may not be the place for the baby’s immature brain to grow correctly,” she added.


Although a Cochrane review of 21 randomised control trials concluded that kangaroo mother care significantly reduces mortality among preterm babies and is a safe and effective alternative to conventional care, global use of the technique remains low. The Every Newborn action plan, endorsed by the World Health Organisation in 2014, set a target to reach at least 50% of the world’s low birth-weight infants with kangaroo mother care by 2020.


Charpak is hopeful that research efforts like the Colombia study will change attitudes to the care of preterm babies, not least among health workers.


“There are barriers related to the implementation of KMC programmes, particularly from health staff,” she said. “We believe long-term results will help convince the doubtful about the benefits of implementing KMC.”

Delay to curbs on toxic shipping emissions "would cause 200,000 extra premature deaths"

A push by the shipping and oil industries for a five-year delay to curbs on toxic sulphur emissions would cause an extra 200,000 premature deaths from lung cancer and heart disease, according to an unpublished International Maritime Organisation (IMO) study.


Fatalities from illnesses such as asthma were not covered by the leaked paper, which was based on shipping satellite data and modelling work.


The shipping industry is by far the world’s biggest emitter of sulphur with SOx levels in heavy fuel oils up to 3,500 times higher than those in current European diesel standards for vehicles. A single large cruise ship can reportedly burn as much fuel as whole towns, and emit more sulphur than 7m cars.


At the end of October, an IMO meeting in London will decide whether to cap the sulphur content of shipping fuels by 2020 or 2025. Current levels can reach 3.5% but the cap would limit them to 0.5%.


The 2020 deadline faces fierce resistance from the oil and gas industry association, IPIECA, and Bimco, a global shipping group, which argue that there is not enough low-sulphur fuel available to meet the global demand that the measure would spur.


The EU has thrown its weight behind 2020, unilaterally imposing the new IMO standard from then. With China enforcing similar emissions control zones, the new benchmark for 2020 is thought likely to pass, although the US and large flag states’ positions remain wildcards.


James Corbett, one of the report’s lead authors, told the Guardian that any slippage on the 2020 start date risked grave consequences.


“An IMO policy implemented on time in 2020 could reduce the health burden on coastal communities, particularly in Asia, Africa and Latin America,” he said. “The inverse is also true. A delay would ensure that health impacts from sulphur emissions persisted in coastal communities that are exposed, where shipping lanes are most intense and communities most densely populated.”


Egypt, Panama, Japan, India, Singapore, the Philippines and China would be among the countries hardest hit, Corbett added.


Sveinung Oftedal, Norway’s lead negotiator at the IMO, said that domestic health concerns had now overtaken fears about the acidifying effects that sulphur has on Scandinavia’s lakes and rivers.


“Air pollution from shipping is not just a local or regional, but a global, problem,” he said. “The question is whether we can really continue to accept its effects, and the answer is: no, we cannot. The 2020 deadline is needed and it is achievable.”


But resentment in the beleaguered shipping trade is unmistakeable, and a sign of future fights to come in the IMO over shipping’s CO2 emissions, which roughly equal those from aviation.


“Most shipping companies are not turkeys voting for Christmas,” an industry source said. “They’re under the impression that a decision to delay to 2025 would be of collective economic benefit. In the real world, shipowners are bleeding money. There is total depression in the industry.”


Thousands of container ships belonging to Hanjin, south Korea’s biggest shipping firm, were left adrift on the world’s oceans carrying a £14bn cargo, after the firm filed for bancruptcy last month.


The International Chambers of Shipping (ICS) says that imposing the new benchmark in 2020 would cost industry an additional $ 50-100bn a year, owing to the cost of low-sulphur fuels.


Ships today mostly run on cheap blends of the residues and remainders left over from the refining and distillation of crude oil for aircraft jet fuel and automobile diesel.


Bimco argues that a premature move to low-sulphur fuels could have knock-on inflationary effects for other fuels, as increased diesel demand overwhelms supply.


Lars Robert Pedersen, Bimco’s deputy secretary-general, told the Guardian: “Quite frankly, if the IMO decides to ignore these very concrete facts, ships will start to use other fuel streams and there will then obviously be a shortage in those streams and a potential disruption in the flow of energy to supply world markets.”


Corbett estimates the total cost of the measure at $ 30bn, compared to a value of seaborne trade approaching $ 5tn in the South China Sea alone. Supporters of an early sulphur cap say that its effect on commodity prices such as shoes or bananas would be little more than a few cents.


Crucially, a second IMO report seen by the Guardian finds that enough refineries will be available in 2020 to guarantee the future availability of a low-sulphur fuel supply for all the world’s ships.


“In all scenarios, the refinery sector has the capability to supply sufficient quantities of marine fuels with a sulphur content of 0.5% m/m or less … to meet demand for these products, while also meeting demand for non-marine fuels,” it says.


A capacity shortage in 2020 would be the only grounds for IMO delegates to delay the proposal, under its terms of reference, which some fear could then be extended indefinitely.


In a reflection of differing opinions in the sea freight trade, the ICS is neutral on the timeline for phasing out sulphur, while calling for a speedy resolution of the issue.


The oil industry too is ambivalent. One analyst with close knowledge of the issue said: “Major oil companies such as Shell and BP don’t have a problem with a move to cleaner fuel in the shipping sector because they have advanced refineries which could sell higher value fuels and increase their revenues and potentially, their profits.”


Later this month, a global CO2 data collection system for ships will be launched. Neither shipping nor aviation firms are covered by the Paris climate agreement, although the ICS has called for the UN’s climate pledging system to be extended to the industry.
