Doctor admits misleading medics over Pauline Cafferkey temperature

A doctor has admitted misleading other medics by concealing Pauline Cafferkey’s raised temperature before she became seriously ill with Ebola.

Hannah Ryan, who volunteered in Sierra Leone in her first year after graduating from medical school, was one of the medics who assessed Cafferkey following the Scottish nurse’s return to the UK in 2014.

Ryan wrote down a temperature 1C lower than it actually was during a “chaotic” screening process at Heathrow airport on 28 December 2014, a medical practitioners tribunal heard on Monday.

A raised temperature can be the first sign of Ebola, which can kill within five days. Cafferkey, who twice nearly died from the virus, went on to develop one of the worst cases on record for people treated in the west.

Cafferkey was cleared of misconduct over the recording of her temperature in September.

The tribunal in Manchester heard on Monday that Ryan recorded Cafferkey’s temperature as being 37.2C despite knowing it was at least 38.2C – above the average body temperature of 37C and higher than the 37.5C threshold requiring further assessment by a consultant in infectious diseases.

She later told another doctor there were no abnormalities in the temperatures of Cafferkey’s group of returnees, according to a written summary of the allegations by the General Medical Council.

Dr Bernard Herdan, the tribunal chair, was told Ryan’s conduct was “misleading and dishonest” and that her “fitness to practise is impaired because of [Ryan’s] misconduct”.

Ryan admitted misleading others and “acquiesced” in the wrong temperature being given but denies misconduct by her actions at the airport and during a subsequent investigation by Public Health England.

Fraser Coxhill, representing the General Medical Council, said Ryan and Cafferkey were one group among many British medics who put their own lives at risk by volunteering their medical skills and going to west Africa to help fight the outbreak.

Deployed on 22 November 2014, they were based at an 80-bed treatment centre in Kerry Town, working “tirelessly in dangerous and highly pressurised conditions” during which they “formed a strong bond of friendship”, Coxhill said.

But about a month later when they returned to the UK on the afternoon of 28 December, the Ebola screening area at the Heathrow terminal was “crowded, noisy and chaotic”.

In the queue to get clearance from PHE medics to be allowed to leave “murmurings of discontent and frustration” grew, the tribunal heard. There were concerns some Scottish medics would miss connecting flights to Glasgow due to delays in the screening process.

Trying to help PHE staff with the process, they agreed to take and record their own temperatures. Ryan took Cafferkey’s temperature, which was 38.2C – a warning sign for the Ebola virus.

The two medics and another nurse with them, Donna Wood, discussed the reading, “during which someone said, ‘Let’s get out of here’,” Coxhill told the tribunal.

Cafferkey’s temperature was then recorded as 37.2C, the form was passed to PHE staff and the medics went on their way.

However, in baggage reclaim there was further discussion between the medics, and PHE staff were contacted.

When Cafferkey’s temperature was taken again it was below the threshold. However, by this time she had taken paracetamol, which lowers body temperature. Cafferkey returned to Glasgow but the next day fell seriously ill with Ebola.

Four days later, Dr Nick Gent, from PHE, called Ryan to investigate what had happened at the airport. Ryan later admitted not telling him she had taken Cafferkey’s temperature and that it was above the threshold for possible Ebola infection.

Coxhill added: “Whilst there is no doubt that Dr Ryan is a practitioner of hitherto unblemished character who undertook important selfless work in Sierra Leone, it is submitted that the events of 28 December 2014 and 2 January 2015 appear to demonstrate someone whose first instinct is to mislead and be dishonest.”

Ryan’s involvement emerged during a misconduct hearing for Donna Wood, another volunteer medic who was suspended for two months in November after being found to have concealed Cafferkey’s raised temperature.

In evidence to the Nursing and Midwifery Council, Ryan said she was in shock after taking Cafferkey’s temperature in her left and right ears and finding it raised.

She told the NMC hearing in a written witness statement: “I asked Pauline if she was feeling OK. She said she was feeling fine.

“I stood there in shock. It was like I was paralysed. I had no clear thought process. Ebola is such a horrible disease that every time you have a high temperature you worry, even when you know there’s no reason to.”

Ryan said only the three medics were present and that Wood “broke the inertia by saying something like, ‘I’m just going to write it down as 37.2 degrees’” so they could “get out of here and sort it out”.

The tribunal is expected to last 10 days.

An official in charge of overhauling China’s organ transplant programme has said the country may still be using organs from executed prisoners in some cases, even though there is technically zero tolerance for the practice.

The admission by Huang Jiefu, a former Chinese deputy health minister, came as human rights activists and medical ethics experts voiced strong objections to his inclusion at a Vatican summit designed to tackle illicit organ trafficking.

The activists said that by giving Huang a platform, the Vatican risked giving China’s practices an air of legitimacy. Huang told reporters on Tuesday that the controversy was “ridiculous” and repeated assertions that the use of organs from prisoners is now “not allowed”.

“There is zero tolerance. However, China is a big country with a 1.3 billion population so I am sure, definitely, there is some violation of the law,” he told reporters at a conference in Rome.

Pope Francis has called illicit organ trafficking a form of modern slavery. At the start of the conference on Tuesday participants painted a bleak picture of the scale of the problem, with patients who are desperate for life-saving procedures flocking to countries like Egypt, India, and Mexico to buy organs cheaply.

Huang, who has long been a controversial figure in the world of transplantation, said trafficking could be stemmed through the creation of a global task force headed by the World Health Organisation.

But experts have questioned Huang’s assessment of the situation, saying China probably still systematically uses the organs of executed prisoners in order to meet an overwhelming demand.

Last year, China’s alleged use of prisoners’ organs was debated at an international conference after two doctors said it was premature to declare China an ethical partner in the international transplant community.

Nicholas Bequelin, the east Asia director for Amnesty International, said it was known at the time that the vast majority of organ transplants in China came from executed prisoners.

The number of prisoners China executes annually is a state secret, but Bequelin said estimates ranged from 3,000 to 7,000. He said experts had cast doubt on Huang’s claims that China had outlawed the practice. “They haven’t stopped the practice and won’t stop. They have a need for organ transplants that far outpace the availability of organs,” Bequelin said.

Details of the process are grim. Bequelin said China did not adhere to World Health Organization recommendations on how doctors should determine whether a person is legally dead. In some cases, organs have been removed before the prisoner would be considered medically dead by international standards.

“The timing of the execution is – we think – sometimes dependent on the need of a particular transplant surgery. You will execute this person at this time on this day, because that is when the patient has to be ready,” Bequelin said. “It is very secret and there is not a lot of reliable information.”

The Vatican has released new bioethics rules that say organ transplantation must involve the free consent of living donors or their representatives and that in ascertaining the death of a donor, it must be diagnosed with certainty, especially when a child is involved.

The broadcaster Mark Austin has revealed how he struggled to understand his daughter’s anorexia and “failed utterly to grasp that she was seriously mentally ill”.

In a candid account, he admits he thought her “crass, insensitive, selfish and pathetic” and became so frustrated he once told her: “If you really want to starve yourself to death, just get on with it.”

But now the TV presenter hopes to break the taboo around potentially deadly eating disorders and is calling for improved mental health provision to deal with the crisis of more than 850,000 young sufferers, predominantly girls, in the UK.

Austin’s daughter Maddy, now 22, became ill in 2012, losing four stone and changing from a healthy and promising athlete to an “emaciated, ghostlike figure”.

He recalled: “I didn’t understand it at first. Cancer I understand … but this was my daughter wilfully destroying herself by not eating.”

Writing in the Sunday Times magazine, he described how Maddy would lie about how much she had eaten and “explode with rage” when challenged.

“She showered me with contempt. As a father you have to make a decision and I made the wrong one. I decided to go on the attack.”

He said: “I even remember saying, ‘If you really want to starve yourself to death, just get on with it’. And at least once, exasperated and at a loss, I think I actually meant it.”

The newsreader acknowledged that as a father he felt “excluded and hated” and found it hard to talk about issues of body image and weight control.

“I floundered and, in the process, ended up poisoning her against me further,” he said.

Austin described how things hit rock bottom for Maddy after a failed spell at a private inpatient unit on a regime of forced feeding. She resisted the treatment and threatened to kill herself. Maddy’s mother became so worried that when her daughter returned home she took time off work and slept on her bedroom floor so as to monitor her around the clock.

Maddy was eventually “saved” by a local NHS day-patient unit.

She recalled: “Eventually it was a local NHS nurse who really understood me and saw the Maddy without the demons. I was lucky, but mental health treatment should not be a lottery.”

Now her father is urging people to take the illness more seriously and is calling for “walk-in centres on the high street of every town and city in this country, manned by trained counsellors”.

He added: “As a country our response is bordering on the pathetic. It is a mental illness, but, almost uniquely, it is one that kills.”

One in 100 women aged between 15 and 30 are affected by anorexia and it is reckoned that one in five chronic anorexics will die as a result of the condition or because they take their own life.

The Centers for Disease Control and Prevention (CDC) finally admitted that products using oil of lemon eucalyptus can provide longer lasting protection against mosquitoes, just as the toxic chemical bug repellent DEET does.

Clinton campaign admits "we could have done better" handling pneumonia news

Hillary Clinton’s campaign has acknowledged mishandling news of her pneumonia, as Donald Trump sought to capitalize on growing questions over his opponent’s trustworthiness at a critical moment in their race for the White House.

“We could have done better yesterday,” wrote Clinton communications director Jennifer Palmieri on Monday, after some Democrats began questioning whether the campaign had been fully transparent in its weekend accounts of her health.

“In retrospect, we could have handled it better in providing more information. That’s on us. We regret that,” added press secretary Brian Fallon.

Clinton was filmed losing her footing and being assisted into a waiting van after leaving early from a memorial for 9/11 victims in New York on Sunday.

Initially, campaign aides said she had “overheated”, though Clinton later insisted “I’m feeling great, it’s a beautiful day in New York,” after she left her daughter’s apartment, where she was taken to rest.

But once the video footage emerged – which appeared to support eyewitness accounts of a more serious incident – the campaign issued a short statement from a doctor revealing she was being treated for pneumonia.

Clinton had been diagnosed on Friday but the condition went undisclosed despite the campaign chastising reporters who had questioned bouts of coughing at recent public events.

The campaign now says Clinton will release “additional medical information” from her doctor in the next few days “to further put to rest any lingering concerns”, but insists there are “no other underlying conditions, the pneumonia is the extent of it”.

“I expect by the middle or back end of the week she will be back out there on the campaign trail,” Fallon told MSNBC. “If it was up to her she would be travelling to California today but it was her doctor’s advice [to rest at home].”

However, even if Clinton does bounce back quickly, the incident is raising fresh questions over trust which could cause more lingering political complications.

“Antibiotics can take care of pneumonia. What’s the cure for an unhealthy penchant for privacy that repeatedly creates unnecessary problems?” wrote Barack Obama’s former adviser David Axelrod.

His tweet prompted Palmieri’s first acknowledgement of regret, in which she added: “But it is a fact that [the] public knows more about HRC than any nominee in history”.

Their public exchange was seized on by the Trump campaign, which said it demonstrated a familiar pattern of secrecy by the Clintons.

“It’s incredibly important to be forthcoming,” said the Republican’s campaign manager Kellyanne Conway. “If you have a diagnosis of pneumonia, just be honest about it when you’re saying you’re overheating. Just say, ‘by the way, I’m on antibiotics’.”

Trump immediately claimed he would soon be releasing the results of a recent physical examination of his own, telling interviewers: “It’s interesting because they say pneumonia, but she was coughing very, very badly a week ago … It’s very interesting to see what’s going on.”

Clinton’s campaign and its backers nonetheless pushed back against criticism regarding transparency. Trump, they pointed out, had thus far declined to release his tax returns and refused to offer policy specifics on issues ranging from immigration to the fight against Islamic State.

The state of Trump’s own health was also unclear, with a brief statement from his personal doctor of 25 years in December providing few medical details and serving as the only record offered by his campaign.

The doctor’s statement described the Republican nominee in the sort of hyperbolic language typically associated with Trump, declaring him “the healthiest individual ever elected to the presidency” if successful in his pursuit of the White House. Trump’s physician, Dr Harold Bornstein, said last month he put the document together in five minutes while awaiting a limo sent by Trump to collect the letter.

Despite repeatedly criticizing Clinton’s “stamina” in recent weeks, Trump was cautious on the topic of the former secretary of state’s health on Monday, saying in an interview: “I hope she gets well soon.”

The Republican nominee instead focused his fire on Clinton’s comments on Friday about half of Trump’s voters being in “the basket of deplorables”. Although she has since expressed regret for being “grossly generalistic” in saying “half”, she still stood by her characterization of Trump supporters. Clinton has continued to reiterate: “Trump has built his campaign largely on prejudice and paranoia and given a national platform to hateful views and voices.”

The Trump campaign has seized on these remarks and is now airing a television ad highlighting them in several swing states. The ad includes footage of Clinton grouping “the racists, sexists, homophobic, xenophobic, Islamophobic, you name it” into the basket of deplorables.

The stumbles for Clinton come as polls appear to show Trump reducing a previously large gap, to an average of three points.

Even as her campaign sought to rein in any political damage from the past few days, several Democrats argued that the focus on Clinton’s health had been overblown by the media.

“Every candidate I have ever worked for has gotten sick on the trail and worked through it because you can’t take days off in a close race,” wrote Dan Pfeiffer, a former senior advisor to Obama.

Bill Burton, who served as Obama’s national press secretary in 2008, said campaign aides would have been “understandably skittish about making announcements about secretary Clinton’s health” while Trump was pushing conspiracy theories about her wellbeing.

“Will it feed a narrative? Sure,” he said in an interview. “But only because the media gets led around by the nose by Donald Trump.”

And while the media was quick to cast the incident as problematic for Clinton, Burton said it could in fact provide her with a small boost in the polls.

“People respond when they think someone’s being treated unfairly,” he said.

“For Hillary Clinton to have pneumonia and even still give a press conference, convene a national security meeting, attend a memorial service in the hot sun for an hour and a half, and then take criticism for having pneumonia all the while … I don’t think the American people are going to punish her for actually performing quite well in the face of what’s an exhausting illness.”

A Massachusetts woman who withheld cancer medication that doctors said could have saved her autistic son’s life was freed on Wednesday after pleading guilty to attempted murder.

Kristen LaBrie, 44, was sentenced to time already served at a change of plea hearing in Salem superior court that ended the decade-long case.

LaBrie, of Salem, did not speak to reporters after Wednesday’s hearing, but her lawyer said it was time to move on. “She’s going to have to live with this for the rest of her life,” said John Morris, who was not her trial attorney.

LaBrie was sentenced to eight to 10 years in prison after her conviction.

Her son, Jeremy Fraser, was diagnosed with a form of non-Hodgkin’s lymphoma at seven and died in 2009, aged nine.

In 2011, she was convicted in 2011 of attempted murder as well as assault and battery and reckless child endangerment, and spent five years behind bars. In March the state’s highest court granted her a new trial on the attempted murder charge after ruling her trial attorney was ineffective because he failed to consult an independent oncologist to try to rebut the prosecution’s claim that LaBrie withheld her son’s medication because she wanted to kill him.

The supreme judicial court also threw out the assault and battery charge and upheld the reckless endangerment charge.

She was released from prison in April.

Testifying in her own defense, she said she largely followed doctor’s orders at first, but acknowledged that she stopped giving him the at-home medications because she couldn’t bear to see how he suffered from the side-effects, which she thought would kill him.

LaBrie was divorced from Jeremy’s father, Eric Fraser, who died in a motorcycle crash just a few months after his son.

Essex district attorney Jonathan Blodgett said on Wednesday the case was always about justice for Jeremy.

“We are satisfied that the sentence was fair, balanced, reasonable, and was tempered with mercy,” he said.
