Rural deprivation and ill-health in England "in danger of being overlooked"

Rural communities are in danger of being overlooked when it comes to poverty, deprivation and ill-health, warns a new report from Public Health England.


In England 9.8m people – or 19% of the population – live in rural settings ranging from coastal villages to market towns to large, open expanses like the Yorkshire Dales.


But experts caution that while on average people in rural areas have better health than those in cities, the popular idea of affluent bucolic life is a misconception.


Instead, they say, official statistics have failed to capture the reality that many residents scattered across rural communities face deprivation, poverty and poor health.


“Within a rural community there are significant inequalities between parts that actually are very wealthy and parts that are quite deprived,” said Dr Rashmi Shukla,
Public Health England’s regional director for the Midlands and East of England and lead author of the report.


Published by the Local Government Association (LGA) and Public Health England, the research sheds light on a number of issues affecting the health and wellbeing of rural communities, including low-paid work, unemployment of young people, high costs of housing and fuel poverty.


Access to health services is also of concern, the authors note, with GPs, dentists and other services further away than in urban areas. Indeed, while 97% of urban households lie within 8km of a hospital, the figure for rural households is just 55%.


In addition, rural areas often lack of public transport, while poor broadband and mobile phone network availability hinders communication and access to online health services, banking, and shopping.


“Even people who may not be materially disadvantaged may have difficulties connecting and may be more socially isolated as a consequence of that,” said Shukla.


Social isolation, she adds, is a particular problem for older people – a sector of society that is larger in rural communities, with 23.5% of residents over the age of 65, compared to 16.3% in urban areas.


An ageing rural population, the authors point out, brings a number of challenges. These include the fact that older people often have poorer health and greater care needs, issues compounded by the greater distances to healthcare services and poor public transport. “Financial poverty in rural areas is also highly concentrated amongst older people, with around one-quarter of those in poverty in pensioner households,” the authors add.


Izzi Seccombe, chairman of the LGA’s Community Wellbeing board, said that many of the services that could help tackle these issues have been under pressure.


“What local authorities have all seen is a reduction in government grant,” she said. “While they have tried to protect the services for vulnerable people, like social care, some of our other services that we deliver – which can be subsidised transport, can be library services, it can be grants to the voluntary sector – those have been squeezed.”


The report encourages local authorities to take action to improve healthcare for rural areas, urging them to look closely at the particular challenges facing different communities. The authors also highlight successful initiatives including the “Fish Well” health improvement project in Norfolk, that offered quayside health checks, advice and support to fishermen that they would otherwise have found difficult to access.


The authors also highlight the need for official statistics that look more closely at conditions experienced by those living in the countryside, and at different areas within it, pointing out that current approaches don’t pick up small pockets of deprivation, and tend use measures that are more relevant in urban areas, such car ownership.


“A lot more digging needs to be done to understand the level of deprivation within rural communities,” said Shukla. “Just under 10m of our population live in these communities and a proportion of them are under severe hardship. We need to address that,” she added.


Seccombe agrees. “We must not forget our vulnerable people in more isolated communities,” she said.

Parents should use covers on their prams during the school run to protect their infants from air pollution, experts have warned.


Scientists tested the pollution levels inside prams to assess the exposure of infants taken on the school run with older siblings. The researchers found that the fine particle pollution from vehicle exhausts, which is particularly harmful, was higher during the morning journey.


“Young children are far more susceptible to pollution than adults, due to their immature and developing systems and lower body weight,” said Dr Prashant Kumar, at the University of Surrey and who led the new research. “These findings provide an insight for families who walk to and from nursery and primary schools with young children. Essentially, children could be at risk of breathing in some nasty and harmful chemical species.”


“One of the simplest ways to combat this is to use a barrier between the in-pram children and the exhaust emissions, especially at pollution hotspots such as traffic intersections, so parents should use pram covers if at all possible,” he said.


The new study, published in the journal Environmental Pollution, placed detectors for particulate pollution in prams and made 64 journeys to and from schools in Guildford at drop-off and pick-up times. They found that air pollution spiked at road junctions and by bus stands, and that fine particle pollution was higher in the mornings, when the roads are busiest.


“Fine particles show larger health impacts compared to their larger counterparts and at the young age children are more susceptible to particulate pollution, suggesting a clear need for precautionary measures to limit their exposure during their transport along the busy roadsides,” the researchers concluded.


Previous work on whether adults are exposed to less pollution than children, who are closer to the level of exhaust pipes, has produced conflicting results. One study showed children were exposed to twice as much particle pollution, while another found children in buggies were exposed to lower levels of fine particles. The new work found no significant differences.


Levels of particulate pollution in the UK are generally below legal limits, but 40 of the 51 air quality zones in the UK exceed the World Health Organization’s (WHO) guideline limits for fine particulate matter, and the WHO has urged the UK to do more to cut pollution. Earlier this week, the WHO revealed that around the world 560,000 children under five years old die each year as a result of air pollution.


Particulate pollution is estimated to cause a total of 29,000 early deaths in adults each year in the UK. Levels of another key pollutant – nitrogen dioxide – are above legal levels in much of the UK. A recent study commissioned by the mayor of London, Sadiq Khan, showed over 800 schools, nurseries and colleges in the capital alone are in areas with illegally high NO2 levels.


On Thursday, Khan announced the first of 12 “low emission bus zones”, where only the cleanest buses will be allowed to run. The first is along Putney High Street, a notorious pollution blackspot, with others to follow including in Brixton.


Khan, who said the zones represent the most extensive network of clean buses of any major world city, commented: “London’s toxic air is an outrage. [This] will make a big difference to the pollution caused by our public transport system.”


Research published by Kumar’s team in February showed that drivers in London are the commuters least exposed to harmful particulate pollution, when compared with those taking the underground or the bus. “There is definitely an element of environmental injustice among those commuting in London, with those who create the most pollution having the least exposure to it,” he said.

“What I would say to female ecstasy users is that you need to more careful than men,” he said. “Women appear to be more at risk of harm. Everyone has to be careful, but I think women need to pay extra attention to things like how much they are using, how they are mixing, where they are and who they’re with.”


One theory is that MDMA, the active ingredient in ecstasy, causes users’ bodies to retain more water, which in some cases can lead to dangerous brain swelling. Oestrogen, the female hormone, impairs cells’ ability to release water, meaning that women are particularly at risk from the effect.


The warnings come after 10 young women died this year having taken ecstasy pills or MDMA powder – more than double the number last year – as high-strength products swamp the market after a drought that lasted several years.


The effects of MDMA doses are less predictable than other drugs, Winstock said. Even a small dose could, in the wrong set of circumstances, kill a user. However, he was keen to stress that the risk posed by the drug could be minimised by careful use.


With an estimated 200,000 Britons using ecstasy every weekend, overstating the harms of the drug was simply not realistic, he said.


Fiona Measham, a professor of criminology at Durham University, said that whereas previous deaths linked to ecstasy were the result of adulterated pills, problems now were more likely to be the result of users accidentally taking more MDMA than they intended.


Measham is the founder of the drug-testing charity The Loop, which checks the strength and content of drugs handed in by users at nightclubs and festivals across the UK.


She said: “With ecstasy-related deaths approaching the highest they have ever been, alongside some of the highest and most variable strength ecstasy pills in circulation, it’s more important than ever this coming party season to take extra care.”

Glastonbury, like nearly every town and village in the UK, is about to suffer an annual outbreak of flashing. It will start in the high street and spread all over town – maybe even to your garden or front room.


Around three in every 100 people with epilepsy have photosensitive epilepsy. Various types of seizure can be triggered by flashing or flickering light: tonic-clonic, absence, myoclonic and focal seizures – all disabling and some dangerous.


Christmas-tree lights that are sold to the public do not have to comply with health and safety regulations. They can flash at any rate, so they can and do cause people to have seizures.


My 21-year-old child, for one, can no longer go out of the house from November until January because of the danger of suffering seizures as a result of flashing Christmas lights in shops and front gardens. Those seizures may continue for days following just one exposure. Just imagine being locked in your house for three months every year and unable to shop or socialise. Many people assume that epilepsy is now controllable, but 20% of epilepsy patients have intractable seizures – seizures that do not respond to treatment.


So this year, when you switch on your Christmas lights on – please realise that if you set them to flash you are actually ruining Christmas for some. Set them to continuous burn and not flash. Then we can all have a merry Christmas.

A US Food and Drug Administration ruling this month bans the use of triclosan, triclocarban and 17 other antiseptics from household soaps because they have not been shown to be safe or even have any benefit.


About 40% of soaps use at least one of these chemicals, and the chemicals are also found in toothpaste, baby pacifiers, laundry detergents and clothing. It is in some lip glosses, deodorants and pet shampoos.


The current FDA action bans antiseptics like triclosan in household soaps only. It does not apply to other products like antiseptic gels designed to be used without water, antibacterial toothpaste or the many fabrics and household utensils in which antibacterials are embedded. Data suggest that the toothpastes are very effective for people suffering from gum disease, although it is not clear if they provide substantial benefits for those who don’t have gingivitis.


The FDA is currently evaluating the use of antibacterials in gels and will rule on how those products should be handled once the data are in.


Although antibacterials are still in products all around us, the current ban is a significant step forward in limiting their use.


As microbiologists who study a range of chemicals and microbes, we will explain why we don’t we need to kill all the bacteria. We also will explain how antibiotic soaps may even be bad by contributing to antibiotic-resistant strains of bacteria that can be dangerous.


Bacteria are everywhere in the environment and almost everywhere in our bodies, and that is mostly good.


We rely on bacteria in our guts to provide nutrients and to signal to our brains, and some bacteria on our skin help protect us from harmful pathogens.


Some bacteria present in soil and animal waste can cause infections if they are ingested, however, and washing is important to prevent bacteria from spreading to places where they can cause harm.


Washing properly with soap and water removes these potential pathogens.


If soap and water are sufficient, why were antibacterials like triclosan and triclocarban added in the first place?


Triclosan was introduced in 1972. These chemicals were originally used for cleaning solutions, such as before and during surgeries, where removing bacteria is critical and exposure for most people is short. Triclosan and triclocarban may be beneficial in these settings, and the FDA ruling does not affect healthcare or first aid uses of the chemicals.


In the 1990s, manufacturers started to incorporate triclosan and triclocarban in products for the average consumer, and many people were attracted by claims that these products killed more bacteria.


Now antibacterial chemicals can be found in many household products, from baby toys to fabrics to soaps. Laboratory tests show the addition of these chemicals can reduce the number of bacteria in some situations. However, studies in a range of environments, including urban areas in the United States and squatter settlements in Pakistan, have shown that the inclusion of antibacterials in soap does not reduce the spread of infectious disease. Because the goal of washing is human health, these data indicate that antibacterials in consumer soaps do not provide any benefit.


What’s the downside to having antibacterials in soap? It is potentially huge, both for those using it and for society as a whole. One concern is whether the antibacterials can directly harm humans.


Triclosan had become so prevalent in household products that in 2003 a nationwide survey of healthy individuals found it in the urine of 75% of the 2,517 people tested. Triclosan has also been found in human plasma and breast milk.


Most studies have not shown any direct toxicity from triclosan, but some animal studies indicate that triclosan can disrupt hormone systems. We do not know yet whether triclosan affects hormones in humans.


Another serious concern is the effect of triclosan on antibiotic resistance in bacteria. Bacteria evolve resistance to nearly every threat they face, and triclosan is no exception.


Triclosan isn’t used to treat disease, so why does it matter if some bacteria become resistant? Some of the common mechanisms that bacteria use to evade triclosan also let them evade antibiotics that are needed to treat disease. When triclosan is present in the environment, bacteria that have these resistance mechanisms grow better than bacteria that are still susceptible, so the number of resistant bacteria increases.


Not only are bacteria adaptable, they are also promiscuous. Genes that let them survive antibiotic treatment are often found on pieces of DNA that can be passed from one bacterium to another, spreading resistance.


These mobile pieces of DNA frequently have several different resistance genes, making the bacteria that contain them resistant to many different drugs. Bacteria that are resistant to triclosan are more likely to also be resistant to unrelated antibiotics, suggesting that the prevalence of triclosan can spread multi-drug resistance. As resistance spreads, we will not be able to kill as many pathogens with existing drugs.


Antibiotics were introduced in the 1940s and revolutionized the way we lead our lives. Common infections and minor scrapes that could be fatal became easily treatable. Surgeries that were once unthinkable due to the risk of infection are now routine.


However, bacteria are becoming stronger due to decades of antibiotic use and misuse. New drugs will help, but if we do not protect the antibiotics we have now more people will die from infections that used to be easily treated. Removing triclosan from consumer products will help protect antibiotics and limit the threat of toxicity from extended exposure, without any adverse effect on human health.


The FDA ruling is a welcome first step to cleansing the environment of chemicals that provide little health value to most people but pose significant risk to individuals and to public health. To a large extent, this ruling is a victory of science over advertising.

Unlike Juvederm, Restylane, Perlane, and other commonly used hyaluronic acid products, which are FDA-approved for cosmetic use, Expression is being used off-label. Nevertheless, in the past couple of years Expression has come into wider use by doctors and cosmetic clinics.

In February, 2014 the FDA contacted Enhancement Medical regarding adverse events reported by Expression users, and in June, 2014 the agency sent Expression a warning letter criticizing the company’s handling of adverse events reported to the agency. The agency criticized Enhancement Medical specifically for closing complaint cases without adequate follow-up to make sure users’ problems were addressed.


Calls and emails to Enhancement Medical were not returned by the time of publication.


One of the properties of hyaluronic acid fillers that makes them so popular is that they are considered to be reversible. An injection of hyaluronidase dissolves the filler. On its website, Enhancement promises:


“When working with Hyaluronic Acids, reversibility becomes one of the most important characteristics. It is essential to know your HA can be reversed if necessary or desired. Using hyaluronidase, the cross-linking is reversed allowing the body to metabolize the gel in just 48 hours.”


In complaints and incident reports, however, people who used Expression describe serious problems that did not resolve with hyaluronidase treatment.


The website RealSelf, which publishes user reviews of cosmetic procedures and products, has a forum for Expression in which users have been posting complaints, concerns and warnings for more than a year. Read back further in the forum, and you’ll see doctors recommending Expression and touting its benefits.


The lesson: If you are interested in using fillers, or just learning more about them, be sure to do careful research. The American Society of Plastic Surgeons has a good round-up on the various fillers available and what they’re best used for.


The FDA’s warning about Expressions is one more example of why beauty consumers – like health care consumers – need to do their own vetting before making decisions about a medical or cosmetic procedure. While we all want to trust our doctors and other professionals – and in most cases we should – you’re safest going the extra mile to find out as much as you can.
