Rural deprivation and ill-health in England "in danger of being overlooked"

Rural communities are in danger of being overlooked when it comes to poverty, deprivation and ill-health, warns a new report from Public Health England.


In England 9.8m people – or 19% of the population – live in rural settings ranging from coastal villages to market towns to large, open expanses like the Yorkshire Dales.


But experts caution that while on average people in rural areas have better health than those in cities, the popular idea of affluent bucolic life is a misconception.


Instead, they say, official statistics have failed to capture the reality that many residents scattered across rural communities face deprivation, poverty and poor health.


“Within a rural community there are significant inequalities between parts that actually are very wealthy and parts that are quite deprived,” said Dr Rashmi Shukla,
Public Health England’s regional director for the Midlands and East of England and lead author of the report.


Published by the Local Government Association (LGA) and Public Health England, the research sheds light on a number of issues affecting the health and wellbeing of rural communities, including low-paid work, unemployment of young people, high costs of housing and fuel poverty.


Access to health services is also of concern, the authors note, with GPs, dentists and other services further away than in urban areas. Indeed, while 97% of urban households lie within 8km of a hospital, the figure for rural households is just 55%.


In addition, rural areas often lack of public transport, while poor broadband and mobile phone network availability hinders communication and access to online health services, banking, and shopping.


“Even people who may not be materially disadvantaged may have difficulties connecting and may be more socially isolated as a consequence of that,” said Shukla.


Social isolation, she adds, is a particular problem for older people – a sector of society that is larger in rural communities, with 23.5% of residents over the age of 65, compared to 16.3% in urban areas.


An ageing rural population, the authors point out, brings a number of challenges. These include the fact that older people often have poorer health and greater care needs, issues compounded by the greater distances to healthcare services and poor public transport. “Financial poverty in rural areas is also highly concentrated amongst older people, with around one-quarter of those in poverty in pensioner households,” the authors add.


Izzi Seccombe, chairman of the LGA’s Community Wellbeing board, said that many of the services that could help tackle these issues have been under pressure.


“What local authorities have all seen is a reduction in government grant,” she said. “While they have tried to protect the services for vulnerable people, like social care, some of our other services that we deliver – which can be subsidised transport, can be library services, it can be grants to the voluntary sector – those have been squeezed.”


The report encourages local authorities to take action to improve healthcare for rural areas, urging them to look closely at the particular challenges facing different communities. The authors also highlight successful initiatives including the “Fish Well” health improvement project in Norfolk, that offered quayside health checks, advice and support to fishermen that they would otherwise have found difficult to access.


The authors also highlight the need for official statistics that look more closely at conditions experienced by those living in the countryside, and at different areas within it, pointing out that current approaches don’t pick up small pockets of deprivation, and tend use measures that are more relevant in urban areas, such car ownership.


“A lot more digging needs to be done to understand the level of deprivation within rural communities,” said Shukla. “Just under 10m of our population live in these communities and a proportion of them are under severe hardship. We need to address that,” she added.


Seccombe agrees. “We must not forget our vulnerable people in more isolated communities,” she said.

Psychopathology

In a study done by Kahn-Greene, et.al, (2007), participants had an increased amount of self-reported psychopathology symptoms after no sleep for two nights. The findings of this study suggest that the loss of sleep can disrupt regions of the brain that involves affective regulation, therefore, serving as a model of how brain dysfunction is connected to psychopathology.


Cardiovascular, Inflammatory, & Metabolic Implications


Sleep curtailment has increased among Americans overs the past few decades, causing it to be associated with the risk of diabetes and obesity (Knutson, et.al, 2007). In today’s society, more than around 30% of men and women, from the age of 30 and 64 years old, report less than six hours of sleep every night (Knutson, et.al, 2007). In a study done by Knutson, et.al, (2007), evidence done from laboratory studies show that there is an adverse reaction from loss of sleep on glucose regulation. This increases the risk between the duration of sleep loss and any existing diabetic condition.


Research is increasing each day on how sleep loss can be linked to cardiovascular risks such as blood pressure, hormonal regulation, cardiac morbidity, and glucose metabolism. In a combination of many self-reported sleep loss complaints, several conclusions were found: short periods of sleep deprivation causes an increase in coronary heart disease, glucose tolerance damage, and an increased risk of diabetes for women (Mullington, et.al, 2009). Within the first stages of sleeping, anabolic hormones, for instance, growth hormone levels reach their maximum. When sleep deprivation is present, this sleep associated growth hormone is either diminished or decreased.


Alertness & Cognitive Performance


Two functions are decreased when sleep deprived are cognitive performance and alertness (Thomas, et.al, 2000). Over the past hundred years, the effects of long-term sleep deprivation in humans have been examined. Decreased amounts of alertness are found in both long term, and short-term sleep loss in individuals. Cognitive performance, with or without the inclusion of lack of alertness, is decreased as well in both long, and short term sleep deprivation (Thomas, et. al, 2000). Therefore, task performances such as decision-making, judgment, logical reasoning, memory, communication, and creative thinking are impaired. These effects can begin as early as the first night of sleep loss, and become worse as time goes on.


Thomas, et. al, (2000), conducted a study that examined the global and regional changes in brain activity that is caused by sleep deprivation. The researchers used a time series design on 17 male volunteers from 21 to 29 years of age. Several types of instruments were used to analyze the subjects and their decreased sleep patterns over several days. The following results were found that just losing one night of sleep decreases waking regional brain activity that mediates alertness and high-order cognitive processes. The impairments following the loss of sleep for one night suggest that just one night of adequate sleep will restore and sustain normal behavior and brain activity. Thus, the findings from this study show that sleep is a necessity for humans to have normal brain functioning. Visual Impairments


Loss of sleep can cause a disruption of the retinal image formation, therefore causing the perceived images to become dim, including double vision. After just one night of losing sleep, being able to perceive stimuli within the peripheral and central areas of the visual field are disruption, and this is increased if driving (Orzel-Gryglewska, 2010). However, driving while sleep deprived is also age-dependent. Driving that is long and monotonous can cause vision to become narrower, whereas severe drowsiness causes a disruption within the entire visual field. Restricting sleep can risk driving safety, showing a correlation in the amount of time and quality of sleep a driver receives and the amount of road accidents (Orzel-Gryglewska, 2010).


During one study, participants that had sleep loss had increased driving errors in a driving simulator than those that didn’t have any. Several variables impact the way sleep deprivation causes driving impairments: age, time of day, the amount of sleep loss, and type of vehicle. Age is a large factor in how drowsiness and sleep deprivation related accidents. This is also determined by the time of day the accident occurred. Drivers that are under the age of 25 tend to cause twice as many road-related accidents when sleep deprived than the majority of driver age groups.  Whereas, older drivers tend to drive during the day with sleep deficiency, and younger drivers cause more accidents at night when sleep deprived (Orzel-Gryglewska, 2010).


How Does this Affect REM Sleep?


REM, rapid eye movement, is a stage of sleep where dreams occur, promoting the encoding of previous memories (Van Heugten- Van, et. al, 2013). Researchers have recently proposed that sleep loss plays a large role in the introduction of dissociative symptoms. Dreamlike mentation can cause implications that invade the waking state, failures in memory, and lead to these dissociative symptoms as well. Through most recent studies, it is clear that the hypotheses that sleep deprivation increased excessive or out of the phase REM activity that causes underlying dissociative symptoms.

Junior doctors" sleep deprivation poses threat to patients, says GMC

Trainee doctors in the NHS are often so sleep-deprived that they are in danger of harming patients, the medical profession’s regulator has said.


Increasingly heavy workloads and widespread staff shortages mean the UK’s 54,000 junior doctors are being left to look after wards of patients without proper experience, according to the General Medical Council’s biggest annual survey of trainee medics’ experiences


One in four doctors below the level of consultant say their schedule leaves them sleep-deprived and 43% describe their workloads as heavy or very heavy. Those with the most intense schedules are much more likely to encounter patient safety being put at risk, the survey found.


In one unnamed hospital, a junior doctor was the sole medic left in charge of 300 patients overnight – a case the GMC’s chief executive, Charlie Massey, said was extremely concerning. The risk of the doctor being unable to deal with two medical emergencies happening simultaneously meant such lack of cover should never happen, he said.


“Patients who are in hospital overnight are in a hospital for a good reason and if something should happen to one or more patients during that nightshift, and that trainee has been left alone, that creates real potential risk to patients,” Massey said.


He said he was “astonished” to have learned that a single doctor had been left unsupervised and having to handle the care of so many patients.


The proportion of trainees describing themselves as sleep-deprived rose from 21% in 2012 to 24.4% this year. “This is an increasing and worrying trend and it’s very concerning when doctors say that it’s jeopardising their ability to make good judgments and to provide safe care,” Massey told the Guardian in his first interview since taking up his role last month.


“Sleep deprivation matters because [tired] doctors may not remember all of the things that they should remember – for example, all the things to do to safely intubate an individual – because they are so knackered or may not remember all the patient’s history and may therefore make the wrong clinical judgment about them.”


Massey is so worried by the potential for patients being harmed by rising fatigue and workload pressure among hospital doctors that he has written to every NHS provider of care across the UK warning them to take steps to ensure the safety and quality of care are protected.


“Those responsible and accountable for the delivery of medical education locally must take the appropriate steps to ensure the training of doctors remains protected, particularly as medical training is so often a bellwether for the quality and safety of patient care, and because patients are directly at risk if the support and supervision of doctors in training is inadequate,” he wrote in the letter.


A separate survey of junior doctors, undertaken by the Royal College of Physicians and published on Wednesday, found that 80% of trainee medics say their job sometimes or often causes them excessive stress.


One in four of the 498 junior doctors surveyed by the RCP said their job seriously affected their mental health and 54% said it affected their physical health. Pressure on trainees had reached a “harmful and unsustainable level”, the college said.


Massey said the demands being placed on trainees were now so great that there was a risk of some being “used and exploited” by their hospitals because there were too few staff to cope with the work that needed to be done.


“We have seen some signs that that is beginning to happen,” he said. The GMC, which supervises all doctors’ training, fears that some are not getting the training they need and the next generation of consultants may not be as highly skilled as they need to be.


Massey also voiced unease that the quality of handovers from one set of doctors to another – which are integral to patients receiving good care – had emerged as a growing concern. Young doctors’ clinical education is also being interrupted by them being called out of training sessions to attend to problems involving patients, the GMC found.


Key medical specialties that have seen some of the sharpest rises in demand from patients – such as A&E, acute internal and general medicine, respiratory medicine and gastroenterology – are those where trainee doctors’ workloads have increased significantly in recent years, the GMC found. A total of 53,835 junior doctors – just under 99% of the 54,563 across the UK who were eligible to complete the GMC survey – took part.


Dr Pete Campbell, of the British Medical Association’s junior doctors committee, said: “Patients and the public may be shocked by these findings, but no junior doctor will be surprised. It is still far too common that junior doctors are left sleep-deprived after regularly working beyond their rostered hours, on rotas that are desperately short of doctors.”


NHS Employers said the new contract being imposed on junior doctors in England, which prompted the recent year-long industrial dispute between the BMA and the government, would remove or mitigate many of the concerns behind their rising dissatisfaction.


“We want to support our junior doctors,” a Department of Health spokesman said. “That’s why the NHS has employed 11,900 more doctors since 2010. Yesterday, the health secretary announced plans to improve junior doctors’ training, including more support from consultants, more notice of future placements including where couples are placed, reviewing the appraisals process and investing £10m to bring doctors back up to speed when they take time out to have a family or other caring responsibilities.”

Sensory deprivation is an aspect of psychological study popularised between the 1950’s and 1970’s. While early sensory deprivation experimentation was done mostly with what is now scientifically called chamber-REST, meaning resting in a bed or chair within a sound free and light proof room, the aspect of recent research has been with the newly popularised floatation-REST method.

While these short sessions of time spent in sound and light proof rooms were shown to induce feelings of relaxation and calmness, floatation-REST produced these effects to a much greater level, with added clinical significance in areas such as experiences of euphoria, enhanced mindfulness, deep meditation and extreme relaxation.


Studies upon floatation have found a wide range of health benefits, even beyond that of the psychological realm, from which floatation tanks were initially created for. Floatation is known to reduce the effects of muscle stress, muscle fatigue, chronic fatigue, along with chronic pain symptoms. After a series of between 3 and 10 regular weekly sessions, these benefits, both physical and mental, were found to be prolonged and enhanced in a wide variety of independent studies.


Sensory deprivation tanks have also been found to alter brain wave signals significantly, inducing the brain into a theta state within 15 to 45 minutes after first entering the tank. Theta brain wave states are known to be experienced in the phase just before falling asleep, and are associated with states of lucid dreaming, hypnosis and deep meditation. Normally the body will experience this state just before entering into sleep, for around 15 minutes, and between phases of REM sleep stages, however these normally last for around 5 minutes or so before changing frequency to a more alert state.


Sensory deprivation tanks on the other hand, have been shown to induce theta brain wave states for the majority of a session, even in longer sessions of 2 to 5 hours, theta brain waves have been measured as the primary dominant brain wave patterns.
