Unproven alternative medicines recommended by third of Australian pharmacists

Nearly one third of pharmacists are recommending complementary and alternative medicines with little-to-no evidence for their efficacy, including useless homeopathic products and potentially harmful herbal products.


The finding comes from a Choice survey of 240 pharmacies including Priceline, Chemist Warehouse and Terry White. Mystery shoppers were sent in to speak to a pharmacist at the prescription dispensing counter and ask for advice about feeling stressed.


Three per cent of the pharmacists recommended homeopathic products, despite a comprehensive review of all existing studies on homeopathy finding that there is no evidence they work in treating any condition.


Twenty-six percent recommended Bach flower remedies to shoppers, homeopathic solutions of alcohol and water containing diluted flower essences. The solution was invened by a British homeopath, Edward Bach, who claimed to have a psychic connection to plants. A comprehensive review of all existing studies on Bach flower solutions found no difference between the remedies and placebos.


The Choice survey also found products containing a B group vitamin complex were recommended by pharmacists for stress in 46% of cases. Other frequently recommended products were St John’s wort and valerian. There is no good evidence that these products reduce stress.


Fifty-nine per cent of people were just told the complementary and alternative product recommended to them worked, and 24% were told the product was scientifically proven, without any evidence being provided to them, the survey found.


A prominent public health expert and the director of Medreach, Dr Ken Harvey, famously quit his job as an adjunct professor with Victoria’s La Trobe University in 2014 after the university struck a $ 15m, six-year deal with vitamins manufacturer Swisse.


He said the Choice survey demonstrated that some pharmacists were failing in their professional duty to consumers.


“Pharmacists are giving crazy advice, and it is dangerous in some cases,” he said.


“My view is that pharmacists, if they are going to sell these products, need to have a big shining sign over the shelves of the complementary and alternative medicine section that says ‘these products have not been assessed by the government regulators to see if they work, please talk to pharmacist’.


Harvey said it was not uncommon for manufacturers of complementary and alternative medicines to do deals with pharmacists so that when customers filled a script for a prescription medicine, the pharmacist would recommend a “front of shop” product to “complement” the prescription medication.


“Pharamacists are giving poor advice and they clearly have a conflict of interest,” Harvey said.


“There is no doubt about that companies. Vitamin companies come into their stores and stick their products on the shelves and provide the pharmacist with posters and pamphlets, and the pharmacists wash their hands of it. But they’re abdicating their professional responsibilities.”


National president of the Pharmacy Guild, George Tambassis, said people trusted pharmacists, and therefore they had a duty-of-care to be aware of available clinical evidence about the therapeutic and marketing claims made about products sold in their stores.


In a statement, the Guild said it was “essential that consumers have access to objective, informed advice about complementary medicines, and by far the best place to obtain that advice is at a community pharmacy”.


Pharmacology lecturer Ian Musgrave from the University of Adelaide said while he expected shop assistants might not be up to date with the latest evidence for complementary and alternative products, he expected better from pharmacists, who have at least four years of university education.


“I would suggest that we need more education, potentially post-graduate education, on herbals and complementary medicines, especially seeing as now a number of pharmacies have a substantial section with these products,” he said.

What You Need to Know About Auvi-Q, the New EpiPen Alternative

Here’s some welcome news for anyone with severe allergies longing for another option besides the famously overpriced EpiPen: Auvi-Q is coming back on the market in the United States in the first half of 2017, according to its maker, Kaléo.

Still, as a parent of a nut-allergic child (one who weirdly enough had the Auvi-Q when it was recalled), my worry is: Am I sure I can trust this product to deliver the correct dose? 


That’s the question I put to the inventors of Auvi-Q, twin brothers Evan and Eric Edwards, an engineer and physician who are VPs with Kaléo. After telling me about their state-of-the art manufacturing process, Evan shares the company’s backstory: “My brother Eric and I are severe allergy sufferers and parents of allergic children. We take the quality and manufacturing of this product very, very seriously because it’s personal for us.”

In fact, it was their own experience growing up allergic to almost everything that drove them to invent the device, Eric adds. “Evan and I, when we were 3, were labeled as some of the most allergic children our physician had ever encountered. We grew up allergic to all eggs, peanuts, tree-nuts, seafood, fish, shellfish, most antibiotics, all insect venom.”  


“Back in the 80s, there was not a lot of awareness. We were were those kids that had to sit alone at a table, because there wasn’t a peanut-free zone back then.” They had epinephrine with them at all times, but once they were college age, Eric admits, “We struggled to carry it. Our whole goal was to create a different option.” 


Different how? Well smaller, for one: Auvi-Q is the size of a credit card and the thickness of the average cell phone. The sleek design isn’t for looks but rather to increase the odds that it will be carried everywhere. “Unfortunately, I’ve had to respond to my own child having an anaphylactic reaction,” Eric says. “It doesn’t matter what epinephrine device you have, if it’s not readily available with you and it’s not used correctly during that moment, it could be tragic.”

Another safety innovation is a fully-retractable needle, designed to prevent the needle from breaking off and getting embedded in a squirming child’s leg. Other epinephrine auto-injectors are required by the FDA to include safety warnings that this could happen, according to the Edwards brothers, but Auvi-Q is not because there have never been any reported cases.

“We recognized the urgent need for a less-expensive epinephrine auto-injector,” CVS says in a press release. “Patients with life-threatening allergies need immediate access to injectable epinephrine. But over time, the brand-name epinephrine auto-injector pens that make it easy to quickly and safely administer the medication have become increasingly expensive.”


The move comes just days after insurance company Cigna said it was dropping coverage for Mylan’s EpiPens. A Cigna spokesperson, Karen Eldred, applauded CVS’ decision in a statement.


“It is positive news for our customers,” said Eldred said, according to NBC News. “The generic version, available now in pharmacies, has the same drug formulation and device functionality as the branded medication, but at a substantial cost savings.”


Mylan is currently under investigation by Congress for raising the price of EpiPen by 500% since they took over production of the life-saving device in 2007. Amid the outcry, Mylan said they would introduce a generic version of the EpiPen, priced at $ 300 for a two-pack.


Mylan CEO Heather Bresch told Congress that the company “never intended” for the price of EpiPens to rise to this level, and claims the hike is due to taxes.


Mylan’s main competitor, Auvi-Q, had stopped production in 2015 because of a lack of name recognition compared to the EpiPen. The company announced in October that it would resume production, hoping to provide another epinephrine auto-injector at an affordable price.


“This is personal for us,” Eric Edwards, who created the Auvi-Q with his twin brother after growing up with severe allergies to nuts, eggs and shellfish, told Forbes. “I’m a patient who may have to respond with this product not only for myself but for my child, and we are committed to making sure that Auvi-Q is going to work during an allergic emergency.”

Fewer than half of the patients with type 2 diabetes have their disease well controlled.  However, there is a need for new, affordable, and effective medications — without unfavorable side effects.  Estimates are that up to 49 percent of diabetics have taken dietary supplements or used other forms of complementary and alternative medicine in an effort to treat their diabetes or other health condition.
