The lesson of Prince Harry’s grief? We need mental health services for all | Suzanne Moore

One of my strongest memories of attending Princess Diana’s funeral is of sitting up high in Westminster Abbey and looking down on her young sons. During the long service no one touched them or hugged them, though everyone was moved by the handwritten “Mummy” card on top of her coffin. The much reviled Sarah, Duchess of York leaned over to see if they were OK. Otherwise, nothing. They were to be “brave”, I suppose. They had walked behind that coffin for a very long way, their heads bowed, impassive. Why had they been made to do that?


The dysfunction of the first family was challenged and disrupted at the funeral by Earl Spencer’s speech. The public grief was dismissed by much of the establishment – as Diana was herself – as hysterical, and it proved the polar opposite of the royal reaction. The family holed up in Balmoral. Charles could now work to get everyone to accept Camilla as his partner, which they largely did. And the boys? It seemed they must just get on with it.


Twenty years later, in a lovely interview with Bryony Gordon in the Telegraph, Prince Harry has now spoken about his grief and how it affected his mental health. The stiff upper lip and the repression required to keep it so is not the way forward. Acknowledging the problems and talking about them is.


The loss of his mother when he was 12 was at the root, Harry says, of his feeling of anxiety, which manifested itself in “fight or flight” responses when in public. He talks of his aggression – he found boxing helped with this – and the feelings of internal chaos and breakdown that come after a deliberate shut-down of emotion. He didn’t want to think about his mother as it would be too sad. Yet, of course, not thinking about loss does not mean its effects go away. Harry was supported by his brother, William; “shrinks” helped as well; and finally, being open helped too.


Seeing fellow soldiers with PTSD from serving time in Afghanistan made him realise that actually this was not the root of his mental health issues. Rather, he had pushed away emotions, shoved them right down, always saying life was fine when it wasn’t at all. “And then I started to have a few conversations and actually, all of a sudden, all this grief I had never processed started to come to the forefront and I was like, ‘There is actually a lot of stuff that I need to deal with.’”


Alongside this, he, William and Kate saw that in much of their charity work, mental health was intrinsically bound to many other issues, but not always addressed. They decided to work on this.


Harry has rightly been praised for talking personally and thus destigmatising mental health issues. This is no doubt excellent. The normalising of mental health problems, which it is estimated will affect a quarter of us at one time or another, is necessary, but so too is funding. Mental health services are in a very poor state and it is almost impossible to get help. Many people in Harry’s situation would not get access to counselling and would be offered antidepressants and possibly a short course of cognitive behavioural therapy, as this is considered most cost-effective. In acute cases, people in a state of severe breakdown are now forced to go to hospitals far from their homes because there are no beds to be found nearby. This is a real crisis, and it is more visible by the day on our streets.


Royals doing charitable work can help, but we need to think about how the whole of society copes with an epidemic of mental health problems in our young people.


Harry’s likeability and frankness, alongside William’s, have helped to rehabilitate the royals’ image: a facade of modernisation. Remember that at the time of their mother’s death the family was viewed as out of touch, anachronistic. Their lack of emotional engagement seemed to belong to another era. Charles’s treatment of Diana, and her refusal to accept the script that had been written for her, threatened their popularity. Lessons were learned and the princes have been part of this PR.


Indeed, Harry is not the first royal to go public with such issues, although I am very glad he has. His mother talked, after all, of her struggles with bulimia and her suicide attempts. She spoke of difficulties with self-esteem and how bulimia gave the illusion of comfort, followed by disgust. She talked of feelings of self-revulsion and the compulsion to “dissolve like a Disprin” back in 1993. Many dismissed her then as unstable, attention–seeking and manipulative as a result.


Twenty years after her death, the fact that her youngest child can open up in such a way and be widely praised for it shows, I hope, that the world has moved on. Now it’s time to make the help and support that was available to a prince available to all.

How will Brexit impact the NHS? It already has. Nurses from the EU are much less keen to come and work here. Today the Times reported that in the last four months of 2015, an average of 797 EU nurses per month signed up to work in the UK; over the same period last year, that number fell to 194 a month. We currently have a huge shortage of nurses, with 24,000 jobs unfilled in England alone.


You can see this shortfall any time you go into a hospital. There is pressure on beds, on doctors and on nurses. Often there just aren’t enough of them, and they are overstretched. Some A&E departments that used to have 20 nurses are now down to half that number, and staff feel at breaking point. The five Cs that nurses are taught in their training – commitment, conscience, competence, compassion and confidence – are impossible to practise properly in the circumstances many are working in.


There have long been warnings of these shortages but a complete lack of long-term planning, now combined with Brexit, is bringing the situation to a head. We have an ageing population with complex needs. One in three nurses is set to retire in the next decade. The introduction of loans instead of bursaries for training means a 23% drop in applications for nursing and midwifery. Anyone could have predicted that taking away bursaries would prompt this result. No one goes into nursing for the money, so why did George Osborne decide to make it even harder? Why, when we need more nurses, are we not reinvesting in training?


This shortage had been plugged by about 7% of our nurses coming from the EU. If we fail to train nurses that’s how it has to be. This is where so much Brexit rhetoric falls apart. While Theresa May talks tough, refusing to guarantee EU nationals a right to stay, these nurses feel neither wanted nor welcome and will understandably go elsewhere. As Janet Davies, chief executive and general secretary of the Royal College of Nursing, has said: “The government is turning off the supply of qualified nurses from around the world at the very moment the health service is in a staffing crisis like never before.”


When in hospital we are at our most dependent. There may be those who complain about immigration but most people have experience of being cared for by someone who has come here to work. Our NHS could not function otherwise. Is May going to further exacerbate the nursing shortage by making the UK so hostile and unwelcoming to EU nurses they will go elsewhere? Well, this is already happening; people won’t take jobs here in such uncertain times. Yet we continue not to train enough nurses and have made it more financially difficult for them. Where is the joined-up thinking on this? The toxic discourse about “foreigners” stealing jobs may not have been intended for the nurse who washes you after your operation, but that’s how it pans out. We can choose to make people feel welcome or not, but it turns out we need them more than they need us.

There wasn’t exactly a muted reaction today to the news that your breakfast may kill you. Trump may start a war on facts, a missile may go astray, but it turns out that the final straw is telling people that their toast is bad for them. According to the Food Standards Agency (FSA), burnt toast is potentially bad, as are crisps, and well-browned roast potatoes. With this news, something has stirred deep within us. You will only take away our delicious crispiness by prising it from our cold dead hands. We will fight for the right to fluff: that is, to increase the surface area of potatoes before roasting them in order to maximise the brown, crispy bits. Isn’t that Jamie Oliver’s contribution to humanity? We don’t want warm bread, we want actual toast, toasted. Don’t even start me on crisps.


But the latest abomination, according to the FSA, is that we are consuming too much acrylamide. This has been shown to increase the risk of cancer in mice and therefore is likely to have the same affect on us. It is a chemical produced by cooking starchy foods at high temperatures. Which is, of course, the miracle of deliciousness that we must now shun, or moderate – if not give up altogether.


Actually, none of this information is new. We knew in 2002 that burnt foods suspected of being carcinogenic, and we’ve since learned that the process of curing is considered risky. We are now told that sugars – basically, what carbs turn into – are the work of the devil, so we shouldn’t be eating them crisped to perfection anyway. But hey, life too short and all that …


Or we could be making it so. The problem with all this is partly to do with our understanding of risk, which is extremely poor, and partly to do with the message everywhere now that cancer is reducible to lifestyle. Scientists have to give us the information they have. But we need more context, or otherwise their studies just become a list of things that are bad for us, because … rat studies. The mature response would be to eat less of the bad things and give up all that is lovely. Live a life of vegetables and the odd nut. Clearly most of us never reach this level of maturity. And anyway, I don’t buy this whole “you are what you eat” argument.


Cancer is not a choice or simply a result of diet. This is a vast over-simplification. We can say that two-thirds of cancers are related to lifestyle “choices”: obesity, smoking and drinking primarily, but a third aren’t. We have to take in genetic and environmental factors. What Susan Sontag described as “disease-producing lifestyles” we could now say is basically modern life. There are carcinogens in the environment as well as those we choose to put into our bodies. Pinning cancer to environmental factors is extremely difficult because epidemiologists rarely get the chance to link individual cancers to specific pollutants.

So the lay person has to wade through this advice on eating without comparative studies, and without fully understanding the risks. Yes, burnt toast might up your risk of cancer. The proponents of this talk as if we are all only individually responsible for our own health. They hold up the myth of the ever well, pure, toxin-free body. Obviously they live in a different environment to the rest of us. Planet Denial.


Of course we can all take better care of ourselves. Eat more veg and steam more stuff. Moderation and all that. Crisped-up carbs may indeed be the opium of the masses, and the masses will hold on to them. We may seek to extend, life but someone also has to talk about the point of living. If roast potatoes aren’t part of that, I don’t know what is.
